健脾益气温阳方对糖尿病酮症酸中毒患者血清炎性因子影响的研究

摘　　要：目的观察健脾益气温阳方对糖尿病酮症酸中毒患者血清中炎性细胞因子水平的影响并探讨其作用机制。方法 116例患者随机分为两组,均给予常规治疗方案,观察组常规治疗基础上口服中药汤剂健脾益气温阳方治疗。观察两组治疗前后的临床症状及血清炎性细胞因子水平并进行比较分析。结果两组治疗前空腹血糖(FBG)、胰岛素(FINs)以及胰岛素敏感指数(ISI)水平差别均不大(均P>0.05)。治疗后两组FBG、FINs水平均较治疗前明显降低(均P<0.05),而ISI水平相比治疗前增高(P<0.05);治疗后观察组FBG、FINs相比对照组明显降低(均P<0.05),而ISI则较对照组增高(P<0.05)。两组治疗前C反应蛋白(CRP)、白细胞介素-6(IL-6)以及肿瘤坏死因子-α(TNF-α)水平均差别不大(均P>0.05)。治疗后两组CRP、IL-6以及TNF-α水平均较治疗前降低(均P<0.05);治疗后观察组CRP、IL-6以及TNF-α水平相比对照组均降低(均P<0.05)。与对照组比较,观察组治疗后痊愈人数明显增加,无效人数明显减少(P<0.05)。结论健脾益气温阳方可有效改善糖尿病酮症酸中毒患者的临床症状,降低机体炎症细胞因子水平,对抑制炎症反应有着积极的作用。Objective： To study the influence of Jianpi Yiqi Wenyang Decoction on the levels of inflammatory cytokines in serum in patients with diabetic ketoacidosis. Methods： 116 cases of patients were divided into the observation group and the control group,58 cases in each group. Two groups were given conventional treatment;the observation group were given oral Chinese medicine medicinal of Jianpi Yiqi Wenyang Decoction based on that. The clinical symptoms and serum levels of inflammatory cytokines were compared between the two groups before and after treatment. Results： The levels of FBG and FINs of two groups after treatment were significantly reduced than that before treatment,and the level of ISI was significantly increased than that before treatment（P 0.05）. The comparison between groups showed that the levels of ISI of the observation group after treatment was significantly higher than that of the control group;the level of FBG of the observation group after treatment was significantly lower than that of cortrol group（P 0.05）. The level of FINs was significantly lower than that of the control group（P 0.05）. The levels of CRP,IL-6 and TNF-α in two groups were significantly lower than before treatment（P 0.05）. The comparison between groups showed that the levels of CRP,IL-6 and TNF-α of the observation group after treatment were significantly lower than that of the control group（P 0.05）. Compared with the control group,the recovered patients of the observation group after treatment were significantly increased,and the number of invalid patients was decreased significantly;the differences between groups had statistical significance（P 0.05）. Conclusion： Jianpi Yiqi Wenyang Decoction can effectively improve the clinical symptoms of patients with diabetic ketoacidosis,reduce the body＇s inflammatory cytokine levels,and has a positive effect on Inhibiting inflammatory response.
