推拿结合针刺治疗急性乳腺炎的临床研究

摘　　要：目的观察推拿结合针刺法治疗急性乳腺炎患者的临床疗效及其对血清C反应蛋白(CRP)的影响。方法选择急性乳腺炎郁滞期患者60例,按随机法分为两组。治疗组以推拿结合针刺进行治疗,每日治疗1次。对照组以金黄散外敷患处,每日2次。两组均3 d为1疗程,观察1疗程,分别于就诊日、治疗1疗程结束后观察记录乳房胀痛情况、乳汁分泌情况、乳房肿块大小,每组治疗前后均采用酶速率散射比浊法进行血清CRP的检测,痊愈后的第7天随访(门诊复诊或电话询问),对上述观察指标进行统计学分析,评定治疗效果。结果采用推拿结合针刺对郁滞期乳痈患者进行治疗,结果示治疗组患者的乳房疼痛程度、肿块大小和乳汁分泌情况改善均好于对照组,差异具有统计学意义(P<0.05),且治疗组患者CRP要明显低于对照组,差异具有统计学意义(P<0.05)。治疗组的疗效要明显高于对照组,差异具有统计学意义(P<0.05)。结论推拿结合针刺法能有效改善急性乳腺炎乳房疼痛、红肿、泌乳不畅和淤积肿块等症状,降低CRP水平,且方法简便,疗效好,可以为治疗急性乳腺炎的首选疗法。Objective： To explore the effectiveness and mechanism of massage combined with acupuncture on acute mastitis by observing the clinical effect and the level of C reactive protein（CRP） in serum. Methods： 60 patients with acute mastitis were enrolled in this study,and divided into the treatment group（n = 30） and the control group（n = 30）. The treatment group received massage combined with acupuncture,once per day. The control group took traditional Chinese medicine Huangjinsan,twice per day. The two groups were treated for 3 days as 1 treatment course. The breast tenderness,breast milk secretion and the size of breast lumps were all recorded before and after treatment. The serum CRPof each group was also detected with scattering rate turbidimetry method.The indicators above were analyzed statistically to assess the therapeutic effect after recovery by outpatient de-partment visits and Tel inquiry. Results： The results showed that the breast tenderness,breast milk secretion and the size of breast lumps in the treatment group were better than those in the control group（P〈0.05）. The level of serum CRP of the treatment group was significantly lower than that in the control group（P〈0.05）. The effectiveness of the treatment group was higher than that of the control group obviously（P〈0.05）. Differences above were statistically significant. Conclusion： The method of massage combined with acupuncture on acute mastitis can improved the symptoms of pain,swelling,poor lactation and silt mass effectively,and reduce the level of serum CRP,simple and effective,which can be the first choice for the treatment of acute mastitis.
