摘　　要：目的:探讨Glove法常规器械行单孔腹腔镜胆囊切除术(laparoscopic cholecystectomy,LC)的临床疗效及应用价值。方法:2015年1月至2017年9月采用自制单孔腹腔镜装置、常规腹腔镜器械为67例胆囊息肉、慢性结石性胆囊炎患者行单孔LC。结果:64例成功完成手术,2例因暴露欠佳改行两孔法LC,1例因胆囊动脉出血改行三孔法LC,无一例中转开腹。手术时间30~110 min,平均(56±27)min;术中出血量2~100 ml,平均(15±4)ml。术后1例出现脐下切口脂肪液化,经换药后痊愈;无胆道损伤、术后出血、胆漏等严重并发症发生。住院1~4 d,平均(2.8±1.5)d。结论:对符合条件的患者采用Glove法常规器械行单孔LC,切口美观,无需购置专门器械,卫生经济学效果突出,手术安全、有效、简单易行,具有推广价值。Objective： To investigate the curative effects and applications of Glove single port laparoscopic cholecystectomy with conventional instruments.Methods： From Jan.2015 to Sep.2017,67 patients with gallbladder polyps of chronic calculous cholecystitis underwent single port laparoscopic cholecystectomy using self-made single port laparoscopic Trocar and conventional laparoscopic instruments.Results： Sixty-four patients were successfully operated by single port method,2 cases were operated by two ports method because of poor exposure,1 case was operated by three ports method because of cystic artery bleeding,no patient was converted to laparotomy.The operation time was（ 56±27） min（ range,30-110 min）,the intraoperative blood loss was（ 15±4） ml（ range,2-100 ml）.Fat liquefaction of umbilical incision occurred in 1 case,and healed after dressing change.No bile duct injury,postoperative bleeding,bile leakage or other serious complications were found.The hospital stay was（ 2.8±1.5） d（ range,1 to 4 d）.Conclusions： Compared with traditional three ports laparoscopic cholecystectomy,the Glove single port laparoscopic cholecystectomy with conventional instruments is more cosmetic with smaller incisions.Besides there is no need to purchase specialized equipment,health economics effect is outstanding.This operation method is safe,effective,easy,and has the value of promotion.
