淋巴细胞免疫治疗封闭抗体阴性患者的调查结果分析

摘　　要：目的探讨淋巴细胞免疫治疗(LIT)对封闭抗体阴性患者的临床疗效及安全性。方法对2015年6—7月在本生殖医学中心因封闭抗体阴性行淋巴细胞免疫治疗患者238例进行问卷调查,就患者的一般情况、治疗原因、治疗效果、治疗不良反应及患者心理状态等进行分析。结果调查结果显示,在本院进行淋巴细胞免疫治疗患者,大部分原因为复发性流产(68.07%),其次是2次及2次以上移植失败患者(18.06%)。LIT治疗后封闭抗体第1个疗程转阳94例,第2个疗程转阳20例。有116例在治疗后受孕,其中正常怀孕105例,发生流产8例,出现宫外孕3例。在淋巴细胞免疫治疗过程中,72例患者发生皮肤红肿,41例发生色素沉着,2例发生注射部位水疱,2例出现过敏性反应,12例出现其他不良反应。68.91%的患者对于淋巴细胞免疫治疗后成功妊娠抱有很大的信心。结论淋巴细胞免疫治疗对封闭抗体阴性患者是一种安全、相对廉价、治愈率高的治疗方法,同时也对这些患者起到一定的心理安慰作用。Objective To investigate the clinical efficacy and safety of lymphocyte immunotherapy（ LIT） in patients with negative blocking antibodies.Methods Patients with negative blocking antibody of 238 cases who underwent lymphocyte immunotherapy at the reproductive medical center from June2015 to July 2015 were interviewed. The general conditions,treatment reasons,treatment effects,adverse reactions,and psychological status of patients were investigated and analyzed. Results The survey results showed that the majority of patients with lymphocyte immunotherapy in our hospital had recurrent spontaneous abortion（ accounting for 68. 07%）,followed by patients with 2 or more transplant failures（ accounting for 18. 06%）. After the LIT treatment,blocking antibody of 94 cases was converted to positive state in the first course,and in the second course of therapy 20 cases were converted to positive state. There were 116 cases of pregnancy after treatment,including 105 cases of normal pregnancy,8 cases of abortion,and 3 cases of ectopic pregnancy. In the course of lymphocyte immunotherapy,skin swelling was observed in 72 patients,pigmentation occurred in 41 cases,blisters occurred in 2 cases,and allergic reactions occurred in 2 cases,other adverse reaction were observed in 12 cases of patients. A total of 68. 91% patients had great confidence in successful pregnancy after lymphocyte immunotherapy. Conclusion Lymphocyte immunotherapy is a safe,relatively cheap treatment with high cure rate for patients with negative blocking antibodies. It also provides some psychological comfort for these patients.
