原发性高血压合并非糖尿病患者血浆肿瘤坏死因子-α水平变化与胰岛素抵抗的关系

摘　　要：目的探讨原发性高血压合并非糖尿病患者中肿瘤坏死因子(Tumor necrosis factor-α,TNF-α)与胰岛素抵抗的关系。方法我院2017年1~12月初次住院的未系统治疗的原发性高血压非糖尿病患者40例和35例健康志愿者,比较两组体重指数(BMI)、胰岛素、空腹血糖、胆固醇、甘油三酯及TNF-α水平,根据稳态模型评估(HOMA-IR)胰岛素抵抗,分析高血压患者生化指标与发生胰岛素抵抗的相关性。结果原发性高血压患者血清胰岛素、甘油三酯、TNF-α水平、显著高于正常血压对照组。原发性高血压患者(r=0.65,P<0.005)和血压正常组(r=0.77,P<0.005)中的TNF-α水平与BMI呈均显著正相关。结论原发性高血压患者具有胰岛素抵抗和更高的TNF-α水平,但TNF-α水平与胰岛素抵抗无关。Objective To investigate the relationship between tumor necrosis factor-α（TN F-α） and insulin resistance in patients with non-diabetic primary hypertension.Methods We selected 40 patients with non-diabetic primary hypertensive who were hos-pitalized for the first time in our hospital from January 2017 to December 2017.At the same time,35 healthy volunteers were selected as controls.The body mass index （BMI） , insulin, fasting blood glucose, cholesterol, triester and TNF-α were compared between the two groups.The steady-state model assessment （HOMA-IR） was used to assess the insulin resistance.The relationship between biochemical indexes and insulin resistant was analyzed.Results The levels of serum insulin, triglycerides and TNF-α in the patients were signifi-cantly higher than those in the control group.The level of TNF-a in the patients and the controls were positively correlated with BMI （r=0.65 ,P=0.005 for the patients and r=0.77 ,P=0.005 for normal controls Respectively） .Conclusion Our data show that pa-tients with primary hypertension have insulin resistance and higher levels of TNF-α, but TNF-α levels are not associated with insulin re-sistance.
