摘 要:目的探讨表观弥散系数(ADC)联合磁共振弥散加权成像(DWI)在前列腺增生与前列腺癌中的诊断价值。方法回顾性分析2015年3月至2018年12月我院收治的68例前列腺病变患者临床资料,均行MRI、DWI检查,观察纳入患者影像学表现,比较前列腺增生、前列腺癌ADC值、DWI信号强度、D值、f值,评估ADC联合DWI在前列腺增生与前列腺癌中的鉴别诊断价值。结果与前列腺增生患者相比,前列腺癌患者的ADC值、D值、f值明显低,DWI信号强度明显高(P<0.05);前列腺癌灶结节在T2WI上绝大多数边界不清、毛糙,且形态不规则,T2WI均匀低信号,未见不均匀信号;增生结节边界较清、光整,形态规则,T2WI呈低信号或高低混杂信号;前列腺癌结节DWI呈高信号,ADC图上表现为低信号;增生结节DWI呈等或稍高信号,DWI呈等或稍高信号;ADC联合DWI鉴别诊断前列腺癌、前列腺增生的准确率为高于DWI和ADC(P<0.05)。结论 ADC联合DWI可有效提高前列腺增生与前列腺癌鉴别诊断的准确率,两者联合应用可作为前列腺增生与前列腺癌鉴别诊断的有效方案。Objective To investigate the value of apparent diffusion coefficient (ADC) combined with magnetic resonance diffusion weighted imaging ( MRDWI) in the diagnosis of prostatic hyperplasia and prostate cancer.Methods The clinical data of 68 padents with prostatic diseases who were admitted to our hospitals during the period from March 2015 to December 2018 were retrospec-tively analyzed.All patients underwent MRDWI examination.The ADC value,DWI signal intensity,D value and f value were compared between prostatic hyperplasia and prostate cancer.The value of ADC combined with MRDWI in the differential diagnosis of prostatic hy-perplasia and prostate cancer was evaluated.Results Compared with patients with prostatic hyperplasia, the ADC value, D value and f value of patients with prostate cancer were significantly lower while the DWI signal intensity was significandy higher ( P〈0.05 ) . Pros-tate cancer nodules showed many unclear and coarse boundaries and irregular shape, showing low homogeneous signal on T2WI.The boundaries of most hyperplastic nodules were clear and smooth, showing low signal or high-low mixed signal on T2WI.The prostate cancer nodules showed high signal on DWI and low signal on ADC.Hyperplastic nodules showed equal or slightly high signal on DWI.The accuracy of ADC combined with DWI in the differential diagnosis of prostate cancer and prostatic hyperplasia was higher than that of DWI or ADC alone.Conclusion ADC combined with DWI can effectively improve the accuracy of differential diagnosis of prostatic hyperplasia and prostate cancer.Combined application of the two can be used as an effective way for differential diagnosis of prostatic hyperplasia and prostate cancer.
