3.0T高分辨率MRI结合扩散加权成像在直肠癌术前T分期中的应用研究

摘　　要：目的探讨3.0 T高分辨率磁共振成像(HR-MRI)结合扩散加权成像(DWI)在直肠癌术前T分期中应用价值。方法回顾性分析67例经临床病理证实为直肠癌患者的术前MRI检查资料,并与术后病理结果对比。结果以术后病理结果为"金标准",术前HR-MRI诊断直肠癌T分期总符合率为85.1%,其中T1~T2期的准确度、敏感性、特异性分别为85.07%,83.30%,88.00%,T3期准确度、敏感性、特异性分别为85.07%,84.20%,85.40%,T4期准确度、敏感性、特异性均为100%。术前HR-MRI结合DWI诊断直肠癌T分期总符合率为92.5%,其中T1~T2期的准确度、敏感性、特异性分别为92.54%,92.86%,92.00%,T3期准确度、敏感性、特异性分别为92.54%,89.47%,93.75%,T4期准确度、敏感性、特异性均为100%。结论HR-MRI结合DWI较单独使用HR-MRI更好的评估直肠前术前T分期,可为临床确定手术方案提供更加客观的资料。Objective Tb evaluate the application value of 10 T high resolution magnetic resonance imaging （ HR-MFII） with diffusion weighted imaging （DWI） in preoperative T staging of rectal cancer.Methods The preoperative MRI data of 67 patients with rectal cancer confirmed by clinical pathology were retrospectively analyzed.Results With the results of postoperative pathological ex-amination as golden standard,the total accuracy of preoperative HR-MRI in the diagnosis of T staging of rectal cancer was 85.1% .The accuracy, sensitivity and specificity at T1 -2 stage were 85.07% ,83.30% and 88.00% Respectively.They were 85.07% ,84.20% and 85.40% , respectively, at T3 stage and all were 100% at T4 stage.The total accuracy of preoperative HR-MRI and DWI in diagno-sis of T staging of rectal cancer was 92.5%.The accuracy, sensitivity and specificity were 92.54% ,92.86% and 92.00% Respective at T1 -2 stage.They were 92.54% ,89.47% and 93.75% , respectively, at T3 stage and all were 100% at T4 stage.Conclusion Combination of HR-MRI and DWI used to evaluate preoperative T staging is better than HR-MRI alone.It can provide more objective data for determination of surgical plan.
