预见性护理干预在肝脏肿瘤切除术患者围手术期急性肺部感染中的应用

摘　　要：目的探讨预见性护理干预在肝脏肿瘤切除术患者围手术期急性肺部感染中的应用效果。方法选取2016年9月至2017年9月在我院收治的98例接受肝脏肿瘤切除术治疗的患者作为研究对象,随机将其等分成对照组和观察组。对照组患者给予常规围术期护理,观察组患者在此基础上给予预见性护理干预。比较两组患者术后恢复情况、干预前后心理状态变化、肺部感染发生率。结果观察组患者术后下床活动时间、肛门排气时间、肠蠕动时间均早于对照组,住院时间短于对照组(P <0. 05);干预后两组患者焦虑自评量表(SAS)和抑郁自评量表(SDS)评分均低于干预前且观察组低于对照组(P <0. 05);观察组急性肺部感染发生率低于对照组(P <0. 05)。结论预见性护理干预能预防或减少肝脏肿瘤切除术患者围手术期急性肺部感染发生,改善患者心理状态,促进患者术后恢复,值得临床推广应用。Objective To explore the application effect of predictive nursing intervention in perioperative acute pulmonary infection in patients with liver tumor resection.Methods To compared the status of postoperative recovery, the change of psychological state before and after intervention, and the incidence of pulmonary infection, 98 cases of patients with liver tumor resection admitted in our hospital from September 2016 to September 2017 were observed, and randomly divided into control group and study group with equal number. The control group adopted the routine perioperative care, while the study group adopted the predicative care intervention on this basis.Results The time of getting out of bed , of anal exhaust and of intestinal peristalsis in the study group were earlier than those in the control group, and the hospitalization time was shorter than the control group（ P 〈0.05）.After intervention, the self-rating anxiety scale（SAS）and self-rating depression scale（SDS）scores of the two groups were lower than those before intervention, and those of the study group were lower than those of the control group（ P 〈0.05）. The incidence of acute pulmonary infection in the observation group was lower than that in the control group（ P 〈0.05）.Conclusion Predictive care intervention could prevent or reduce the incidence of acute pulmonary infection in patients underwent liver tumor resection, improve patients’ psychological state and promote postoperative recovery,which was worthy of clinical promotion and application.
