摘  要:目的:探讨通过苦参碱预处理后,对大鼠脑缺血再灌注损伤外周血Th1/Th2细胞亚群及其活化的细胞因子的影响作用。方法:建立大鼠全脑缺血再灌注损伤模型,将50只SD大鼠随机分为5组,分别为假手术对照组、手术模型组、苦参碱(4mg/kg,8mg/kg,16mg/kg)预处理组。手术前48h,假手术对照组与手术模型组大鼠分别腹腔注射等量的0.9%氯化钠注射液,药物预处理组腹腔注射相应剂量的苦参碱溶液,每日2次,每次2m L,共给药2日。于脑缺血再灌注后4h,8h,16h,24h进行股动脉取血流式细胞仪检测T细胞亚群;分离血清,采用ELISA法检测细胞因子(IL-1β、TNF-α、IL-4、IL-10)。结果:苦参碱各预处理后可明显降低外周血CD4^+T细胞水平降低(P<0.05或P<0.01),升高CD8^+T细胞水平(P<0.05或P<0.01),降低Th1细胞活化的促炎细胞因子(TNF-α、IL-1β)的水平(P<0.01或P<0.05),升高Th2细胞活化的抗炎细胞因子(IL-4、IL-10)的水平(P<0.01或P<0.05)。结论:苦参碱通过调节Th1/Th2细胞亚群,调整后Th1/Th2细胞活化的细胞因子水平,减轻炎症反应,达到脑神经缺血再灌注损伤的保护作用。从而起到对大鼠脑缺血再灌注损伤的神经细胞保护作用。Objective: To investigate the effects of Matrine on Th1/Th2 subsets and activated cytokines in peripheral blood of rats with cerebral ischemia reperfusion injury. Methods: 50 SD rats were randomly divided into 5 groups,namely sham operation group,model group,matrine pretreatment group( 4 mg/kg,8 mg/kg,16 mg/kg). 48 hours before operation,rats in both sham operation group and model group were gave saline by intraperitoneal injection,other drug pretreatment group were gaven matrine injection,respectively,bid for 2 days. Cerebral ischemia-reperfusion injury model in rats were established. 4 h,8 h,16 h,24 h later,T cell subsets in femoral artery blood were detected by flow cytometry. Then the serum was separated and the cytokines( IL-1β,TNF-α,IL-4,and IL-10) were detected by ELISA. Results: The matrine pretreatment can significantly reduce the level of CD4+T cells in peripheral blood( P〈0.05 or P〈0.01),increase the level of CD8+T cells( P〈0.05 or P〈0.01),reduce the level of proinflammatory cytokines TNF-α and IL-1β in the activated Th1 cell( P〈0.05 or P〈0.01). Increase the level of anti-inflammatory cytokines IL-4 and IL-10 in the activated Th2 cells( P〈0.05 or P〈0.01). Conclusion: Matrine can play a neuroprotective role on cerebral ischemia reperfusion injury in rats by regulating Th1/Th2 cell subsets,adjusting cytokines in activated Th1/Th2 cell and inhibiting inflammation reaction.
