鼻腔冲洗联合丙酸氟替卡松对变应性鼻炎患者的临床研究

摘　　要：目的:研究鼻腔冲洗联合丙酸氟替卡松鼻喷雾剂对变应性鼻炎患者血清细胞间黏附分子-1(ICAM-1)、嗜酸粒细胞阳离子蛋白(ECP)、白细胞介素-5(IL-5)及骨桥蛋白(OPN)的影响,并分析临床效果。方法:选取2015年5月至2017年5月治疗的变应性鼻炎142例,根据不同治疗方案分为观察组和对照组,对照组给予鼻腔冲洗,观察组在对照组的基础上联合丙酸氟替卡松鼻喷雾剂治疗。治疗后观察两组临床疗效及不良反应,并检测治疗前后血清ICAM-1、ECP、IL-5水平;检测鼻腔阻力和OPN;检测患者体征评分和症状评分。结果:观察组总有效率高于对照组,差异具有统计学意义(P<0.05);治疗后,观察组血清ECP、IL-5均低于对照组,ICAM-1高于对照组,差异具有统计学意义(P<0.05);观察组鼻腔阻力和OPN均低于对照组,差异具有统计学意义(P<0.05);观察组体征评分和症状评分明显低于对照组,比较差异具有统计学意义(P<0.05)。结论:鼻腔冲洗联合丙酸氟替卡松鼻喷雾剂治疗变应性鼻炎,有效改善患者血清CAM-1、ECP、IL-5水平及鼻腔阻力和OPN,减轻患者体征评分和症状评分,提高临床疗效,且不良反应较少。Objective： To study curative efficacy of nasal flushing combined with fluticasone propionate nasal spray in treatment of allergic rhinitis and its effects on ICAM-1 ECP IL-5 and osteopontin,and analyze the clinical effect. Methods： 142 patients of allergic rhinitis received therapy from may 2015 to may 2017 in our hospital were selected as research objects. According to different treatment regimens were divided into observation group and control group,the control group was treated with Nasal flushing,while the observation group was treated with nasal flushing combined with fluticasone propionate nasal spray. Then clinical effects,adverse reactions,serum ICAM-1,ECP,IL-5 levels,nasal resistance and OPN,physical scores,symptom scores were compared,. Results： The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant（ P〈0.05）. After treatment,serum ECP and IL-5 in the observation group were lower than those in the control group,and ICAM-1 was higher than those in the control group（ P〈0.05）. The nasal resistance and OPN in the observation group was less than the control group（ P〈0.05）. Physical scores and symptom scores in the observation group was less than the control group（ P〈0. 05）. Conclusion： Nasal irrigation combined with Fluticasone Propionate Nasal Spray in the treatment of allergic rhinitis,effectively improved the serum CAM-1,ECP,IL-5 levels and nasal resistance and OPN,alleviated the symptoms score and symptom score of the patients,and improved the clinical efficacy,and had less adverse reactions.
