摘 要:目的观察脑苷肌肽注射液联合左卡尼汀注射液治疗新生儿缺氧缺血性脑病(HIE)的临床疗效及安全性。方法将102例HIE患儿随机分为对照组和试验组,每组51例。对照组予以脑苷肌肽每次2 m L,qd,肌内注射;试验组在对照组治疗的基础上,予以100 mg·kg^(-1)·d^(-1)左卡尼汀,bid,静脉推注。2组患儿均治疗14 d。比较2组患儿临床疗效、血清神经胶质酸性蛋白(GFAP)、泛素羧基末端水解酶L1(UCH-L1)、丙二醛(MDA)、超氧化物歧化酶(SOD),以及药物不良反应的发生情况。结果治疗后,试验组和对照组的总有效率分别为78.43%(40例/51例)和49.02%(25例/51例),差异有统计学意义(P<0.05)。治疗后,试验组和对照组的GFAP分别为(0.02±0.01)和(0.05±0.01)ng·m L^(-1),UCH-L1分别为(0.85±0.06)和(1.44±0.13)ng·m L^(-1),MDA分别为(4.39±0.44)和(5.33±0.52)mmol·L^(-1),SOD分别为(114.93±15.46)和(85.02±8.49)U·m L^(-1),差异均有统计学意义(均P<0.05)。2组患者的药物不良反应均以感染、肺出血和消化道出血为主。试验组和对照组的总药物不良反应发生率分别为7.83%和9.80%,差异无统计学意义(P>0.05)。结论脑苷肌肽注射液联合左卡尼汀注射液治疗HIE的临床疗效确切,其能有效降低患儿血清GFAP和UCH-L1水平,缓解氧化应激损伤,且不增加药物不良反应的发生率。Objective To observe the clinical efficacy and safety of cattle encephalon glycoside and ignotin injection combined with levocarnitine injection in the treatment of neonatal hypoxic ischemic encephalopathy( HIE). Methods A total of 102 children with HIE were randomly divided into control and treatment groups with 51 cases per group. Control group was given cattle encephalon glycoside and ignotin 2 m L per time,qd,intramuscular injection. Treatment group received 100 mg·kg^(-1)·d^(-1) levocarnitine, bid, intravenous injection, on the basis of control group. Two groups were treated for 14 days. The clinical efficacy,glial fibrillary acidic protein( GFAP),ubiquitin carboxy-terminal hydrolases L1( UCH-L1), malondialdehyde( MDA), superoxide dismutase( SOD),and adverse drug reactions were compared between two groups.Results After treatment,the total effective rates of treatment and control groups were 78. 43%( 40 cases/51 cases) and 49. 02%( 25 cases/51 cases) with significant difference( P〈 0. 05). After treatment,the main indexes of treatment and control groups were compared: GFAP were( 0. 02 ± 0. 01) and( 0. 05 ± 0. 01) ng · m L^(-1),UCH-L1 were( 0. 85 ± 0. 06) and( 1. 44 ± 0. 13) ng·m L^(-1),MDA were( 4. 39 ± 0. 44) and( 5. 33 ± 0. 52) mmol·L^(-1),SOD were( 114. 93 ± 15. 46)and( 85. 02 ± 8. 49) U·m L^(-1),the differences were statistically significant( all P〈 0. 05). The adverse drug reactions of two groups were infection,pulmonary hemorrhage and gastrointestinal bleeding. The total incidences of adverse drug reactions in treatment and control groups were 7. 83% and 9. 80% without significant difference( P〈 0. 05).Conclusion Cattle encephalon glycoside and ignotin injection combined with levocarnitine injection has a definitive clinical efficacy in the treatment of neonatal HIE,which can effectively reduce the levels of serum GFAP and UCH-L1,and alleviate oxidative stress injury,without increasing t
