益气固表丸对COPD稳定期患者Th17/Treg平衡的影响

摘　　要：目的观察益气固表丸对稳定期慢性阻塞性肺病(COPD)Th17/Treg免疫失衡的影响。方法将125例中医证属肺脾气虚证的COPD稳定期患者按照随机、双盲、对照原则,分为益气固表丸组(n=59)和安慰剂组(n=66),治疗12w后,检测用药前、后肺功能、外周血血清白细胞介素-10(IL-10)、白细胞介素-17a(IL-17a)水平和淋巴细胞Th17/Treg平衡的变化。结果益气固表丸组COPD患者肺功能测定发现用药后第一秒用力呼气量/用力肺活量%(FEV1/FVC%)和第一秒用力呼气量%(FEV1%)升高,差异具有统计学意义(P<0.05),安慰剂组肺功能各项指标均未见明显变化,差异无统计学意义(P>0.05),采用ELSIA方法检测发现益气固表丸组COPD患者外周血IL-17a水平无明显变化,差异无统计学意义(P>0.05),IL-10水平升高,差异有统计学意义(P<0.05),安慰剂组IL-17a和IL-10水平未见升高或降低,差异无统计学意义(P>0.05)。流式细胞技术检测发现益气固表丸组COPD患者用药后外周血淋巴细胞T细胞亚群中Th17%降低,Treg%升高,Th17/Treg水平下降,差异有统计学意义(P<0.05),安慰剂组Th17%、Treg%及Th17/Treg水平均未见明显变化,差异无统计学意义(P>0.05)。结论益气固表丸对COPD稳定期患者外周血中T细胞亚群具有良性调节作用,能够配合西医基础治疗改善COPD患者的免疫状态。Objective To observe the effect of Yiqi gubiao Pill on Th17/Treg immune imbalance in Chronic Obstructive Pulmonary Disease（COPD）patients in stable period.Methods 125 patients with COPD in feipiqixu Syndromes were divided into 2 groups according to the principle of randomized,double-blind,and control：Yiqi gubiao Pill（Yqgb）group（n =59）and placebo（P）group（n =66）.The changes of pulmonary function,peripheral blood IL-10,IL-17 alevels and Th17/Treg balance in peripheral blood lymphocytes were detected before and after 12 weeks.Results The indexes of FEV1/FVC% and FEV1% were higher significantly（P 0.05）in the Yqgb group,and lung function indexes in the P group were unchanged（P 0.05）.The ELISA results showed that IL-17 awas unchanged in the Yqgb group（P 0.05）,and IL-10 was increased in the Yqgb group statistically significant（P 0.05）.In the P group,IL-17 aand IL-10 did not increase or decrease（P 0.05）.The Flow cytometry results showed that,in the Yqgb group,Th17% was decreased,Treg% was increased and Th17/Treg was decreased statistically significant（P 0.05）,respectively.In the P group,Th17%and Treg% were not changed statistically significant（P0.05）.Conclusion Yiqi gubiao Pills have a benign effect on T cell imbalance in peripheral blood of patients with COPD which in a stable period,and can improve the immune status of COPD patients with combination with basic modern medicine treatment.
