N_2O清醒镇痛对高龄患者结肠镜检查的有效性及安全性分析

摘　　要：目的研究低浓度N_2O清醒镇痛在高龄患者结肠镜检查中的安全性和有效性。方法将2012年5月至2016年6月行N_2O清醒镇痛结肠镜检查的520例≥70岁高龄患者作为观察组,同期常规行结肠镜检查的580例≥70岁高龄患者作为对照组,观察2组患者术前、术中、术后心率(HR)、血氧饱和度(SPO2)、血压(BP)情况,以及检查满意率及不良反应发生情况。结果观察组术中HR、SPO2、BP与对照组比较差异有统计学意义(P<0.05),且观察组满意率高于对照组,不良反应明显低于对照组,差异有统计学意义(P<0.05)。结论低浓度N_2O清醒镇痛用于结肠镜检查提高了老年患者结肠镜检查的舒适度和安全性,临床效果良好。Objective To investigate the effectivity and safety of low-dose nitrous oxide（ N_2 O） conscious sedation in colonoscopy for elderly patients. Methods A total of 520 senile patients（ age ≥70 years） who underwent colonoscopy with low-dose N_2 O conscious sedation in our hospital from April 2012 to June 2016 were enrolled as observation group,at the same time,the other 580 senile patients who underwent conventional colonoscopy in our hospital at the same period were enrolled as control group. The heart rate（ HR）,blood oxygen saturation（ SPO2）,blood pressure（ BP）,degree of satisfaction of examination and incidence of adverse reactions before operation,during operation and after operation were observed and compared between the two groups. Results There were significant differences in HR,SPO2 and BP during operation between two groups（ P〈0. 05）. Moreover the degree of satisfaction of examination in observation group was significantly higher than that in control group,and the incidence of adverse reactions in observation group was significantly lower than that in control group（ P〈0. 05）. Conclusion The low-dose nitrous oxide conscious sedation in colonoscopy for elderly patients can improve degree of comfort and safety,with better clinical effects.
