1470 nm环形激光在下肢静脉曲张治疗中的临床体会

摘　　要：目的探讨环形激光(1470nm)在下肢静脉曲张治疗中的临床效果。方法回顾分析本院2016年3月至2017年4月203例行大隐静脉高位结扎、腔内激光闭合的下肢静脉曲张患者(208条患肢)的临床资料与随访信息。结果患者手术均顺利进行,手术时间35~45分钟。208条患肢中有76条发生各种不同程度的并发症,发生率为36.5%,其中出现皮下瘀斑、青紫、肿胀发生率为22.1%,下肢皮肤感觉麻木发生率为6.7%,皮肤烧伤发生率为6.7%,下肢血栓性静脉炎1.9%,下肢深静脉血栓0.9%,均给予对症治疗后痊愈。患者术后随访3~12个月,2例复发,其余患者曲张静脉消失,无水肿,下肢酸胀明显减轻,溃疡创面愈合,色素沉着减轻。结论 1470 nm环形激光在下肢静脉曲张治疗中疗效满意,是一种安全、有效的微创治疗。Objective To investigate the clinical effects of 1470 nm ring laser in the treatment of lower extremity varicose veins. Methods The clinical data of 203 patients （208 limbs） with lower extremity varicose veins treated by great saphenous vein high ligation plus endovenous laser ablation in our hospital between March 2016 and April 2017 were analyzed retrospectively. Results All operations were successful, the average operation time was 35-45 min. Complications occurred in 76 legs （36.5%）. Including subcutaneous stasis, blue and swelling （22.1%）, local skin numbness （6.7%）,skin burnt （6.7%）, thrombophlebitis （1.9%） and deep venous thrombosis （0.9%）. All complications were cured after symptomatic treatment. In the followup of 3-12 months, two patients recurrent. The varicose veins and the felling of swelling disappeared, and the pigmentation and ulcer wound were reduced or disappeared. Conclusions 1470 nm ring laser in the treatment of lower extremity varicose veins is a satisfactory and efficient method, which have advantages of safety, effectiveness and minimal invasion.
