摘 要:目的评价和比较腰围身高比值(WHtR)与体重指数(BMI)筛查心血管风险(2型糖尿病、高血压、高脂血症)的效果。方法采用受试者工作特征曲线(ROC)计算灵敏度、特异度以及确定筛查切点,通过曲线下面积(AUC)比较筛查方法的效果。结果当筛查2型糖尿病(T2DM)时,BMI切点为23.71,WHtR切点为0.5,WHtR筛查效果较好,敏感度为82.6%,特异度为48.6%。当筛查高血压时,BMI切点为24.9,WHtR切点为0.49,WHtR筛查效果较好,敏感度为70.7%,特异度为53.6%。当筛查高脂血症时,BMI切点为24.07,WHtR切点为0.49,WHtR筛查效果较好,敏感度为67.3%,特异度为56.1%。结论与BMI相比,WHtR筛查2型糖尿病、高血压、高脂血症的效果较好,可作为有效的预测心血管风险的中心型肥胖指标之一。Objective To evaluate and compare the effect of waist-to-height (WHtR) and body mass index (BMI) on screening cardiovascular disease risk factors (type 2 diabetes,hypertension,dyslipidaemia). Methods The data of 1662 subjects were stud- ied,measured height,weight,waist and blood pressure,then OGTT was performed and the HbAlc and blood lipid were examined. Receiver operating characteristic curve (ROC) was conducted to calculate sensitivity,specificity and confirm the screening cut- points,the area under the curve was used to compare the effect of screening methods. Results The optimal cutpoints of BMI, WHtR related to type 2 diabetes screening were 23.71,0.5 respectively,the screening accuracy of WHtR was better,with the sensitivity and specificity of 82.6% and 48.6% respectively. The optimal cutpoints of BMI ,WHtR related to hypertension screening were 24.9,0.49 respectively,the screening accuracy of WHtR was better,with the sensitivity and specificity of 70.7% and 53.6% respectively. The optimal cutpoints of BMI,WHtR related to dyslipidaemia screening were 24.07,0.49 respectively,the screening accuracy of WHtR was better,with the sensitivity and specificity of 67.3% and 56.1% respectively. Conclusion As compared with BMI ,the accuracy of WHtR for screening type 2 diabetes, hypertension, hyperlipidaemia is better. This result suggests that WHtR may be an effective index for central obesity predicting cardiovascular disease risk factors.
