全程膝关节功能训练在膝骨性关节炎行关节镜下关节清理术后的应用

摘　　要：目的探讨全程膝关节功能训练在膝骨性关节炎(KOA)行关节镜下关节清理术后的应用。方法选取2016年1月~2017年12月KOA病例共66例,随机分为康复组和对照组。两组病例均行关节镜下膝关节清理术,对照组术后予以常规患肢功能锻炼,康复组术后予以全程膝关节功能训练,两组均干预8周。观察并记录两组术前和术后8周简易Mc Gill疼痛评分及膝关节Lysholm评分的变化,并比较其临床效果。结果术后8周,两组Mc Gill疼痛评分及膝关节Lysholm评分较干预前明显改善(P<0.05或P<0.01),且康复组改善情况较对照组更显著(P<0.05);同时康复组临床优良率(93.94%)明显优于对照组(75.76%)(χ~2=4.24,P<0.05)。结论全程膝关节功能训练在KOA患者关节镜下关节清理术后应用的效果确切,能显著减轻患者疼痛,明显改善膝关节功能。Objective To explore the application of whole course knee functional training after arthroscopic joint debridement in knee osteoarthritis（KOA）. Methods A total of 66 KOA cases from January 2016 to December 2017 were selected and randomly divided into rehabilitation group and control group. Both groups were treated with arthroscopic knee debridement. The control group was given conventional limb functional training after surgery. The rehabilitation group was given whole course knee function training after surgery. Both groups were intervened for 8 weeks. The changes of simple. McGill pain scores and Lysholm scores of knee joints before and 8 weeks after surgery were recorded and their clinical effect was compared. Results The McGill pain score and the Lysholm score of the knee joints were significantly improved 8 weeks after sugery（P〈0.05 or P〈0.01）, and the improvement in the rehabilitation group was more significant than that in the control group （P〈0.05）. The clinical excellent rate （93.94%） of the rehabilitation group was significantly better than that of the control group（75.76%）（χ^2=4.24, P〈0.05）. Conclusion The whole knee functional training is effective after arthroseopic joint debfidement in KOA patients, which can significantly reduce pain and improve knee function.
