摘 要:目的探讨绝经妇女原发性良性阵发性位置性眩晕(iBPPV)与骨代谢异常的相关性。方法选取30例2014年1月~2016年6月期间就诊我院的绝经后iBPPV妇女作为iBPPV病例组,选取30例同期就诊于我院的骨质疏松绝经妇女作为骨质疏松病例组,选取28例同期本院体检中心绝经后健康体检者为对照组。三个观察组分别检测腰椎及股骨颈的骨密度(BMD),以最低T值进行统计学分析对比。同时检测三个观察组的血清骨代谢生化指标Ca、P、碱性磷酸酶(ALP)、骨合成标志物1型前胶原氨基端肽(P1NP)和骨吸收标志物1型胶原羧基端肽β特殊序列(β-CTX),并分别进行统计学分析对比。结果 iBPPV病例组、骨质疏松病例组和对照组的T值比较,差异有统计学意义(P<0.001)。三组Ca、ALP比较差异无统计学意义(P>0.05),P检测值比较差异有统计学意义(P<0.05)。三组P1NP和β-CTX比较,差异均无统计学意义(P>0.05)。非条件式Logistic回归分析表明,T值与iBBPV的发生有关(P<0.05),其余因素(Ca、P、ALP、P1NP和β-CTX)与iBBPV的发生无关(P>0.05)。结论绝经妇女iBPPV与BMD变化存在相关性,但未发现与骨代谢生化指标和骨代谢生物标志物有相关性,提示绝经妇女iBPPV的发病与骨代谢异常具有相关性,研究结果对iBPPV诊断、治疗、预后评定和预防具有一定指导意义。Objective To investigate the correlation between primary benign paroxysmal positional vertigo(iBPPV) and abnormal bone metabolism in postmenopausal women. Methods 30 cases of postmenopausal women with iBPPV in our hospital from January 2014 to June 2016 were selected as the iBPPV case group. 30 osteoporotic menopausal women who were treated in our hospital during the same period were selected as the osteoporosis case group. In the same period, 28 cases of the post-menopausal health checkup at the physical examination center of this hospital were selected as the control group. Bone mineral density (BMD) of the lumbar spine and femoral neck was measured in three observation groups, and statistical analysis was performed with the lowest T value. The bone metabolic biochemical markers Ca, P, alkaline phosphatase (ALP), bone biosynthetic marker type 1 procollagen amino terminal peptide (P1NP) and bone resorption marker type 1 collagen carboxy terminal peptide beta(β-CTX) in three observation groups were simultaneously detected, statistically analyzed and compared. Results There was significant difference in the T value between iBPPV case group, osteoporosis case group and control group(P〈0.001 ). There was no significant difference in Ca and ALP among the three groups(P〉0.05). There was a statistically significant difference of the P values(P〈0.05). There was no significant difference among the three groups in P1NP and β-CTX(P〉0.05). Non-conditional logistic regression analysis showed that T value was related to the occurrence of iBBPV(P〈0.05), and other factors(Ca, P, ALP, P1NP and β- CTX) were not related to the occurrence of iBBPV(P〉0.05). Conclusion There is a correlation between iBPPV and BMD in postmenopausal women. But there is no correlation between iBPPV and biochemical markers of bone metabolism, biomarkers of bone metabolism, suggesting that the onset of iBPPV in postmenopausal women is associated with abnormal bone metabolism.
