肺肾双补膏联合三伏贴对肺肾两虚型慢性阻塞性肺疾病稳定期患者白细胞介素8及T淋巴细胞亚群的影响

摘　　要：目的观察肺肾双补膏联合三伏贴对肺肾两虚型慢性阻塞性肺疾病(COPD)稳定期患者白细胞介素8(IL-8)和T淋巴细胞亚群的影响。方法将120例肺肾两虚型COPD稳定期患者按照随机数字表法分为2组。对照组60例予支气管扩张剂等常规治疗;治疗组60例在对照组治疗基础上加肺肾双补膏联合三伏贴治疗。2组均以1年为1个观察周期。比较2组治疗前后肺功能第1 s用力呼气容积占预计值百分比(FEV1%)、运动能力6 min步行距离(6 MWD)、中医证候积分、1年急性加重住院次数、IL-8及T淋巴细胞亚群CD4^+、CD8^+、CD4^+/CD8^+变化情况。结果治疗后2组FEV_1%及6 MWD均较本组治疗前明显增加(P<0.05),中医证候积分及1年急性加重住院次数减少(P<0.05),且治疗组6 MWD明显高于对照组(P<0.05),中医证候积分及1年急性加重住院次数低于对照组(P<0.05)。治疗后2组IL-8水平及CD8^+均较本组治疗前降低(P<0.05),CD4^+及CD4^+/CD8^+升高(P<0.05),且2组组间比较差异均有统计学意义(P<0.05)。结论肺肾双补膏联合三伏贴治疗肺肾两虚型COPD稳定期疗效显著,可降低IL-8水平,减轻炎症反应,调节机体免疫功能,改善临床症状及肺功能,减少急性加重发作。Objective To observe the effect of Feishen Shuangbu ointment combined with dog days plaster on Interleukin-8 and T Lymphocyte subsets in patients with chronic obstructive pulmonary disease at stable stage in lung ankidney deficiency syndrome. Methods 120 patients with stable chronic obstructive pulmonary disease in Lung and Kidney deficiency syndrome were randomly divided into two groups. 60 patients in control group were treated by conventional bronchodilators. 60 patients in treatment group were treated by Feishen Shuangbu ointment combined with dog days plaster on the basis of the control group. The percentages of forced expiratory capacity （FEV 1%）, 6-minute walking distance （6MWD）, scores of TCM syndromes, hospitalization times of acute exacerbation in one year, changes of IL-8 and T lymphocyte subsets CD4 ＋, CD8 ＋, CD4 ＋/CD8 ＋ were compared between the two groups before and after treatment. Results After treatment, the FEV 1% and 6MWD of the two groups were significantly increased compared with the previous treatment group （ P 〈0.05）, the TCM syndrome score and the 1-year acute exacerbation hospitalization number decreased （ P 〈0.05）, and the treatment group 6MWD was significantly higher than the control group （ P 〈0.05）. After treatment, the levels of IL-8 and CD8 ＋ in the two groups were lower than those in the group before treatment （ P 〈0.05）, CD4 ＋ and CD4 ＋/CD8 ＋ were increased （ P 〈 0.05 ）, and the differences between the two groups were statistically significant （ P 〈0.05）. Conclusion The combination of Feishen Shuangbu ointment and dog days plaster is effective in the treatment of chronic obstructive pulmonary disease in lung and kidney deficiency. It can reduce IL-8 level, reduce inflammation, regulate immune function, improve clinical symptoms and lung function, and reduce acute exacerbation. frequency.
