升清降浊胶囊对慢性肾脏病4～5期伴高尿酸血症患者肾功能及中医症状的改善作用

摘　　要：目的观察升清降浊胶囊对慢性肾脏病(CKD)4~5期伴高尿酸血症(HUA)患者肾功能及中医症状的改善作用。方法将80例CKD 4~5期伴HUA患者按照随机数字表法分为2组。对照组40例予常规治疗;治疗组40例在对照组治疗的基础上加升清降浊胶囊口服。治疗4周后,比较2组治疗前后收缩压(SBP)、舒张压(DBP)、血红蛋白(Hgb)、血肌酐(Cr)、内生肌酐清除率(Ccr)、尿素氮(BUN)、24 h尿蛋白定量(UTP)及中医症状评分变化。结果治疗后治疗组SBP较本组治疗前降低(P<0.05),DBP无明显变化(P>0.05);Hgb较本组治疗前升高(P<0.05),且高于对照组治疗后(P<0.05),差值亦高于对照组(P<0.05)。对照组治疗后DBP、SBP及Hgb均较本组治疗前无明显变化(P>0.05)。治疗后2组Cr、BUN及UTP均较本组治疗前下降(P<0.05),Ccr升高(P<0.05),且2组Cr、Ccr水平及差值组间比较差异均有统计学意义(P<0.05)。2组治疗后中医症状评分均较本组治疗前下降(P<0.05),且治疗组低于对照组(P<0.05),差值亦大于对照组(P<0.05)。结论升清降浊胶囊能显著改善CKD 4~5期伴HUA患者肾功能,降低SBP,增加Hgb含量,改善临床症状。Objective To observe the effect of Shengqing Jiangzhuo capsule on renal function and TCM symptoms in patients with chronic kidney disease（ CKD） in stage 4 to 5 with hyperuricemia（ HUA）. Methods 80 patients with CDK stage 4 to 5 with HUA were randomly divided into two groups according to the random number table method. 40 patients in control group were treated by conventional therapy. 40 patients in treatment group were given oral administration of Shengqing Jiangzhuo capsule on the basis of the treatment of the control group. The systolic blood pressure（ SBP）,diastolic blood pressure（ DBP）,hemoglobin（ Hgb）,serum creatinine（ Cr）,endogenous creatinine clearance（ Ccr）,urea nitrogen（ BUN）,24 h urine protein quantitation（ UTP）,and changes in TCM symptom scores before and after treatment were compared. Results After treatment,the SBP of the treatment group was lower than that of the control group（ P〈0. 05）,and there was no significant change of DBP（ P〈0. 05）. After treatment,the Hgb in treatment group was higher than that before treatment（ P〈0. 05）,and higher than that in the control group（ P〈0. 05）,the difference was also greater than that in the control group（ P〈0. 05）. After treatment,the DBP,SBP and Hgb in the control group were not significantly different from those before treatment（ P〈0. 05）. After treatment,the levels of Cr,BUN and UTP in the two groups decreased（ P〈0. 05）,and the levels of Ccr increased（ P〈0. 05）. There were significant differences in the levels of Cr and Ccr between the two groups（ P〈0. 05）. The scores of TCM symptoms in the two groups were lower than those in the group before treatment（ P〈0. 05）,and the treatment group was lower than the control group（ P〈0. 05）,and the difference was also greater than the control group（ P〈0. 05）. Conclusion Shengqing Jiangzhuo capsule can significantly improve renal function in patients with CKD in stage 4 to 5 with HUA,reduce SBP,increase Hgb content a
