调补阴阳方联合雷公藤多苷片治疗Ⅳ期糖尿病肾病疗效观察

摘　　要：目的观察调补阴阳方联合雷公藤多苷片治疗Ⅳ期糖尿病肾病的临床疗效。方法将88例Ⅳ期糖尿病肾病患者按照随机数字表法分为2组。对照组43例予雷公藤多苷片治疗,治疗组45例在对照组治疗基础上加调补阴阳方治疗。2组均连续治疗4周。比较2组治疗前后血糖[空腹血糖(FPG)、糖化血红蛋白(Hb A1c)]、血脂[总胆固醇(TC)、甘油三酯(TG)]、肾功能[血尿素氮(BUN)、血肌酐(Cr)、尿白蛋白排泄率(UAER)]、24 h尿蛋白定量及氧化应激[丙二醛(MDA)、超氧化物歧化酶(SOD)]相关指标变化,统计比较2组临床疗效及不良反应。结果治疗组总有效率93.33%,对照组总有效率74.42%,治疗组总有效率及临床疗效均优于对照组(P<0.05)。2组治疗后TC、TG、FPG及Hb A1c均较本组治疗前下降(P<0.05),且治疗组低于对照组(P<0.05)。2组治疗后BUN、Cr、UAER及24 h尿蛋白定量均较本组治疗前下降(P<0.05),且治疗组低于对照组(P<0.05)。2组治疗后MDA均较本组治疗前降低(P<0.05),SOD升高(P<0.05),且2组组间比较差异均有统计学意义(P<0.05)。治疗组不良反应总发生率(8.89%)低于对照组(27.91%,P<0.05)。结论调补阴阳方联合雷公藤多苷片治疗Ⅳ期糖尿病肾病可改善患者肾功能,提高临床疗效,减轻氧化应激,降低不良反应发生率。Objective To observe the effects of tripterygium glycosides combined with tonifying Yin and Yang prescription on stage IV diabetic nephropathy. Methods 88 patients with diabetic nephropathy were randomly divided into two groups according to the random table method. 43 cases in control group were treated by tripterygium glycosides tablets. 45 patients in treatment group were treated by tonifying Yin and Yang prescription on the basis of the control group. Both groups were treated for 4 weeks. The blood glucose levels [fasting blood glucose（ FPG）,glycosylated hemoglobin（ HbA 1 c） ],blood lipids [total cholesterol（ TC）,triglyceride（ TG） ],renal function [blood urea nitrogen（ BUN）,serum creatinine（ Scr）,UAE,24 h urine protein quantitation]and oxidative stress [malondialdehyde（ MDA）,superoxide dismutase（ SOD） ] before and after treatment were compared,and the clinical efficacy and adverse reactions of two groups were statistically compared. Results The total effective rate（ 93. 33 %） and clinical efficacy of treatment group were better than control group（ 74. 42 %,P〈0. 05）. The TC,TG,FPG and HbA 1 c were decreased in the two groups after treatment（ P〈0. 05）,and the treatment group was lower than control group（ P〈0. 05）. The BUN,Scr,UAER and 24 h urine protein levels in the two groups were lower than those in the group before treatment（ P〈0. 05）,and the treatment group was lower than the control group（ P〈0. 05）. The MDA of the two groups was lower than that of the group before treatment（ P〈0. 05）,and SOD increased（ P〈0. 05）. The difference between the two groups was statistically significant（ P〈0. 05）. The overall incidence of adverse reaction in treatment group was（ 8. 89%） lower than that in the control group（ 27. 91%,P〈0. 05）.
