摘  要:目的利用速度矢量成像技术联合氨基末端脑钠肽前体(NT-proBNP)研究早发型和晚发型重度子痫前期患者的左室舒张及收缩功能。方法选取早发型重度子痫前期组患者32例,晚发型重度子痫前期组患者35例,正常健康孕妇38例,应用速度矢量成像技术获得左室长轴整体功能相关参数:收缩期应变(S)、收缩期应变率(SRs)、舒张期应变率(SRd),同时测定各组血浆NTproBNP浓度。结果早发型和晚发型重度子痫前期组的S、SRs及SRd均明显低于正常组,以早发型重度子痫前期组更明显(P<0.01)。与正常组相比,早发型和晚发型重度子痫前期组血浆NT-proBNP浓度均明显增高,以早发型重度子痫前期组更为明显(P<0.01)。结论早发型和晚发型重度子痫患者的左室舒张和收缩功能均受损,以早发型重度子痫患者的损害更为严重。速度矢量成像技术较常规超声心功能指标更能早期的评价重度子痫前期患者心功能。Objective To investigate left ventricular diastolic function and systolic function in patients with early-onset and late onset severe preeclampsia by velocity vector imaging(VVI)combined with N-terminal pro brain natriuretic peptide precursor(NT-pro-BNP).Methods About 105 patients were analyzed:35,32,and 38 with late onset severe preeclampsia,early onset severe preeclampsia and the healthy control group,respectively.Conventional echocardiography and VVI were performed and blood levels of NT-pro-BNP were analyzed.Results The values of S,SRs and SRd in early-onset and late-onset severe preeclampsia were significantly lower than those in normal group,especially in early-onset severe preeclampsia group(P〈0.01).Compared with the normal group,the plasma concentrations of NT-pro-BNP in early-onset and late-onset severe preeclampsia groups were significantly increased,especially in early-onset severe preeclampsia group(P〈0.01).Conclusions Left ventricular diastolic and systolic function are impaired in patients with early and late onset severe eclampsia,especially early onset preeclampsia.Velocity vector imaging is more effective in evaluating the cardiac function of patients with severe preeclampsia than conventional echocardiographic indexes.
