摘　　要：目的对早、晚期慢性肾病(CKD)患者采用三维斑点追踪(3D-STI)评价左室局部收缩功能。方法按肾小球滤过率将患者分为CKD早、晚期组及对照组。比较组间各参数的差异,并绘制对照组与早期组、早期组与晚期组热点图评价其差异性。结果 (1)对照组与早期组,前间隔基底段(BAS)、下壁基底段(BI)、间隔心尖段(AS)、侧壁心尖段(AL)应变值于SLS、SRS、SCS、SAS显著减低(P<0.01);(2)早期对照组与晚期组,前间隔中间段(MAS)、下壁心尖段(AI)、心尖部(Apex)应变值在SLS、SRS、SCS、SAS中显著减低(P<0.01),有较多的节段于SLS中有显著性差异;(3)热点图显示:SLS中各参数显著性差异明显。结论 3D-STI能早期评价CKD者左心室局部收缩功能,SLS是较敏感指标。Objective To evaluate left ventricular regional function in patients with early and late stage chronic kidney disease（CKD）by three-dimensional speckle tracking imaging（3D-STI）.Methods According to glomerular filtration rate,patients were divided into early,late CKD group and reference group.The differences between the two groups were compared,and the differences between the normal group and the early group,the early group and the late hot spot map were drawn.Results The normal and early group,anterior septal basal segment（BAS）,inferior basal segment（BI）,septal apical segment（AS）,the side wall of the apical segment（AL）in SLS,SRS,strain SCS and SAS were significantly decreased（P〈0.01）;the early and late middle period（before the interval group.MAS）,inferior apical segment（AI）,apex（Apex）strain significantly decreased in SLS,SRS,SCS,SAS（P〈0.01）,a segment of more than SLS had significant differences;the hot figure shows：the parameters in SLS were significant difference.Conclusions 3D-STI can evaluate the left ventricular regional systolic function in CKD patients at early stage,and SLS is a sensitive index.
