超声在发育性髋关节异常吊带治疗中的应用价值

摘　　要：目的探讨超声检查在发育性髋关节异常吊带治疗中的应用价值。方法超声随访48个接受吊带治疗的异常髋关节,采用Graf方法评估髋关节分型和经腹股沟横切面检查监测股骨头位置,综合两种方法的检查结果,指导临床治疗方案的选择,追踪超声检查结果与X线评估髋关节最终发育情况的一致性。结果 44个异常髋关节在治疗中经腹股沟髋关节横切面超声检查提示股骨头在位,Graf方法分型髋关节发育好转,综合两种检查结果,指导临床持续吊带治疗2～3个月,治疗后X线检查评价的髋关节发育正常。另有4个采用横切面超声检查监测提示股骨头持续不在位,Graf分型髋关节发育恶化,X线检查提示髋关节脱位或半脱位,遂终止吊带治疗方案。所有超声检查结果与X线检查评价髋关节发育情况相符。结论超声检查可以作为随访监测髋关节发育不良患儿吊带治疗下股骨头位置及髋臼形态改变的常规方法,指导临床治疗方案的选择。Objective To investigate the value of hip ultrasonography in the treatment of development dysplasia of the hip with Pavlik Harness.Methods 48 patients diagnosed with DDH（dislocated or dysplasia）were treated by pavlik harness are included in this study.Hip ultrasonography and transliguinal ultrasound were applied during follow up at the same time for evaluating the relationship between the femoral head and acetabulum.The type of Graf modify was recorded.After the patient turned 6 months,pelvic X-rays were taken.Results 44 of 48 patients showed congruency of femoral head and acetabulum through transliguinal sonography at the first followup after wearing Pavlik Harness.With time,the shape of the acetabulum has been improved and keep harness treatment for around 2 or 3 month,according to the hip ultrasonography or pelvic x-rays to decide when to terminate the harness treatment.In 4 of 48 cases,the femoral head continually dislocated after the first 2-3 months of Pavlik harness treatment,and need to withdraw the harness.The result of the ultrasonography and pelvic X-rays are consistent.Conclusions The hip ultrasonography could be routinely used in monitoring the relationship of femoral head to the acetabulum and evaluateing the shape of acetabulum during the pavlik harness treatment.It could be used to guide the selection of clinical treatment plan.
