脑白质疏松症的经颅多普勒超声联合颈部血管彩超研究

摘　　要：目的探讨经颅多普勒超声和颈部血管彩超联合检查在脑白质疏松症(leukoaraiosis,LA)中的应用价值。方法选取郑州大学第二附属医院神经内科收治的脑白质疏松症患者120例为LA组,依据Fazekas评分分为轻、中、重3组;将同时期入院治疗的无脑白质疏松症患者120例设为对照组,所有患者接受经颅多普勒超声和颈部血管彩超联合检查。结果 LA组大脑中动脉(MCA)双侧收缩期血管流速(Vs)降低,搏动指数(PI)增高,颈内动脉斑块数量增多,与对照组比较差异有统计学意义(P<0.05)。LA组MCA的Vs、PI值与颈内动脉斑块数量相关(P<0.05),斑块数量与Vs值呈负相关,与PI值呈正相关。LA组患者病情越严重其MCA的Vs值越低,PI值越高(P<0.05)。结论经颅多普勒超声和颈部血管彩超联合检查在评估LA病情进展及血管情况中有重要价值。Objective To explore the application value of transcranial doppler ultrasonography and cervical vascular color Doppler ultrasonography in leukoaraiosis （LA）. Methods 120 patients with leukoaraiosis treated by Neurology Department of the Second Affiliated Hospital of Zhengzhou University were selectedas group LA,which was divided into mild,middle and serve groups according to the Fazekas score.At the same time,120 patients without leukoaraiosis treated in the hospital were set as control group.All patients received transcranial Doppler ultrasonography and cervical vascular ultrasound examination. Results The bilateral systolic blood flow rate （Vs） of the middle cerebral artery （MCA） in group LA was reduced,the pulsatile index （PI） increased and the number of plaque in the internal carotid artery increased.There was a statistical difference compared with the control group （ P 〈0.05）.The Vs and PI value of MCA in group LA were correlated with the number of internal carotid artery plaques （ P 〈0.05）,and the number of plaques was negatively correlated with Vs value,and positively correlated with PI value.In group LA,the more serious the patients,the lower the Vs value of MCA and the higher of the PI value （ P 〈0.05）. Conclusion Transcranial Doppler ultrasonography and cervical vascular color Doppler ultrasonography have important application value in assessing the progression of LA disease and vascular condition.
