摘 要:目的通过肿瘤圆度量化分析乳腺良恶性肿瘤及不同分子亚型乳腺癌的MRI形态学特征。方法回顾性分析2015年3月至2016年12月行乳腺MRI检查的213例患者资料,测量肿块的肿瘤圆度,比较良恶性肿瘤间肿瘤圆度的差异,并用多元线性回归分析肿瘤圆度的独立影响因素。区分不同分子亚型乳腺癌的肿瘤圆度,通过one-way ANOVA分析比较不同分子亚型乳腺癌及与良性肿瘤间肿瘤圆度的差异。结果 213例患者,通过病理结果确诊乳腺癌150例,乳腺良性肿瘤63例。乳腺癌中Luminal A型43例(28.7%),Luminal B型69例(46%),HER2过表达型16例(10.7%),三阴性乳腺癌(TNBC)22例(14.6%)。良恶性肿瘤间肿瘤圆度的差异有统计学意义(良性75.9%,恶性56.5%,P〈0.001)。肿块大小、分子分型是肿瘤圆度的独立影响因素。不同分子亚型乳腺癌及乳腺良性肿瘤间肿瘤圆度差异显著,良性肿瘤与TNBC组肿瘤圆度明显高于其他三组,而良性肿瘤与TNBC间肿瘤圆度差异无统计学意义(肿瘤圆度平均值良性肿瘤75.91%、TNBC 70.52%,HER2过表达型59.71%,Luminal A型54.68%、Luminal B型52.44%)。结论肿瘤圆度可作为MRI形态学特征的量化指标,TNBC形态学上更倾向于圆形,与良性肿瘤的MRI表现有重叠性。Objective Using tumor circularity( TC) as a quantitative index to evaluate the MRI morphology of benign and malignant breast tumors,as well as different subtypes of breast cancer. Methods From March 2015 to December2016,213 patients had been examined by 3. 0 T MRI scan were collected. TC of breast cancer had been measured and compared to the benign tumor. Multiple linear regressions were used to find the independent influential factors of TC. Molecular subtypes of breast cancer were also classified. The mean TC of each subtype was demonstrated and compared in between groups as well as benign tumor by one-way analysis of variance. Results 150 cases were defined breast cancer while 63 cases as benign tumor by histopathology. In breast cancer,43 cases( 28. 7%) were classified as luminal A subtype,69 cases( 46%) as luminal B subtype,16 cases( 10. 7%) as HER2-overexpressed subtype,and 22 cases( 14. 6%) as triplenegative breast cancer( TNBC). TC was significantly different between breast cancer and benign tumor.( The mean TC of breast cancer as 56. 5%,and 75. 9% for benign tumor) Tumor size and molecular subtypes were independent influential factors of TC. TC of the benign tumor and TNBC were higher than the other three groups with stastistical significance. But it showed no statistically significant difference between benign tumor and TNBC( 75. 91% for benign tumor,70. 52% for TNBC,59. 71% for HER2-overexpressed,54. 68% for luminal A and 52. 44% for luminal B). Conclusion TC can be used to evaluate MRI morphology as a quantitative index. TNBC turned to be more round in MRI features which was similar with benign tumor.
