摘 要:目的探讨3.0 T MR T2^*加权成像(T2^*WI)评估大鼠慢性肝炎的价值。方法皮下或腹腔注射四氯化碳(CCl4)制备大鼠慢性肝炎模型,对模型组及正常对照组大鼠行肝纤维化程度分期(S)、炎症活动度分级(G)和肝脂肪变性分度(F)。在T2^*WI图像上分析大鼠肝脏与肌肉信号强度比值(SIR(肝/肌))与慢性肝炎病理表现的关系。结果 SIR(肝/肌)值与肝纤维化程度负相关(r=-0.622,P=0.000),与肝脂肪分度微弱相关(r=-0.238,P=0.049),和肝炎活动度无关(P=0.413);在肝纤维化分期组间差异均有统计学意义(F=15.129,P=0.000),在肝脂肪分度间差异无统计学意义(F=1.309,P=0.281);肝纤维化程度各组间两两比较,除S2与S3(P=0.320)差异无统计学意义外,其余两组间差异均有统计学意义(P〈0.05)。结论 T2^* WI序列的SIR(肝/肌)值与肝纤维化程度负性相关,与肝炎活动度无关,能较好地进行肝纤维化程度分期。Objective To explore the value of 3. 0 T magnetic resonance T2^* weighted imaging in evaluating chronic hepatitis of rat models. Methods The models of chronic hepatitis were induced in rat models byintraperitoneal and abdominal injection of carbon tetrachloride( CCl4). The stage of hepatic fibrosis( S),grade of inflammation( G) and degree of fatty liver( F) for the chronic hepatitis model animals and their normal controls were evaluated. The relationship between the liver-to-muscle signal intensity ratio( SIR) and pathological features of chronic hepatitis was analyzed by using T2^* weighted imaging. Results The liver-to-muscle SIR of T2^* WI sequence showed a negative correlation withthe stage of liver fibrosis( r =-0. 622,P = 0. 000),a low correlation with the degree of fatty liver( r =-0. 238,P = 0. 049). There wasno relationship between the liver-to-muscle SIR and the grade of inflammation( P = 0. 413). The liver-to-muscle SIRof T2^* WI sequences for all rats including normal control rats in the stage of liver fibrosis from S0 to S4 had a significant difference( F = 15. 129,P = 0. 000). But there was no significant difference in the degreeof hepatic steatosis from F0 to F4( F =1. 309,P = 0. 281). The pairwise comparisons of the liver-to-muscle SIR of T2^* WI sequences between the rest stages were statistically significant( P 0. 05) but the liver-to-muscle SIR of T2^*WI sequences had no significant difference between the S2 and S3 stages( P = 0. 320). Conclusion The liver-to-muscle SIR of T2^* WI sequence is negatively correlated with hepatic fibrosis and not related to hepatitis,and can be used to distinguish the stage of hepatic fibrosis.
