多层螺旋CT评价孤立性腹腔干扩张性疾病

摘　　要：目的探讨多层螺旋CT多种成像技术在孤立性腹腔干扩张性疾病（ICTD）诊断中的价值。方法搜集多层螺旋CT诊断为ICTD 20例影像学资料,采用薄层原始图像结合容积再现（VR）、最大密度投影（MIP）、多平面重组（MPR）、曲面重组（CPR）等后处理技术进行分析。结果 20例ICTD中,腹腔干真性动脉瘤7例,其中1例伴有胃左动脉瘤,1例伴有肠系膜上动脉夹层;腹腔干夹层13例,9例累及腹腔干分支,1例伴有肠系膜上动脉瘤,1例伴有肠系膜上动脉夹层。11例CT平扫提示腹腔干异常征像。多层螺旋CT血管成像（MSCTA）中,真性动脉瘤均显示瘤腔,夹层动脉瘤显示双腔结构、内膜片、破口及血栓,其中4例行DSA检查,2例进行血管腔内治疗。结论CT平扫能提示腹腔干异常征像,有利于ICTD的检出。MSCTA能清晰、准确地显示ICTD的特征性表现,进一步明确其诊断。Objective To evaluate the value of multi-slice spiral CT imaging in the diagnosis of Isolated Celiac Trunk Dilatation（ ICTD）. Methods The clinical imaging data of 20 patients,diagnosed as ICTD via multi-slice spiral CT were analyzed retrospectively. Post-processing techniques,such as volume rendering（ VR）,Maximum intensity projection（ MIP）,multiple planar reconstruction（ MPR） and curved planar reformation（ CPR）,were used for further analysis in combination with the original scans. Results Among 20 ICTD patients,7 cases were diagnosed as splenic artery aneurysms,of which 1 case associated with left gastric artery aneurysms 1 case associated with superior mesenteric artery dissection. 13 cases were diagnosed as abdominal dry dissection,of which 9 cases involving the branch of celiac trunk,1 case associated with superior mesenteric aneurysm,and 1 case associated with superior mesenteric artery dissection. 11 cases of CT scan showed abnormal celiac trunk signs. In the multi-slice spiral CT angiography（ MSCTA）,true aneurysms showed tumor cavity,and the dissecting aneurysms showed double-chamber structure,inner membrane,rupture and thrombus. 4 patients received a DSA examination,and 2 patients underwent endovascular treatment. Conclusion CT scan can indicate the abnomality of celiac trunk and aid in the detection of celiac trunk dilatation. MSCTA can clearly and accurately show the characteristic manifestation of ICTD,and therefore confirms its diagnosis.
