摘  要:糖尿病是血管病变、心室重构及心功能恶化的重要危险因素。在强调心血管获益的糖尿病治疗时代,钠-葡萄糖协同转运蛋白-2抑制剂(SGLT-2i)的发现无疑具有里程碑式的意义。越来越多的证据表明SGLT-2i具有多重的心血管保护作用,SGLT-2i治疗相比于安慰剂可显著改善糖尿病患者的心血管预后。令人欣慰的是,SGLT-2i能有效减少患者因心力衰竭发作再住院的风险,这一发现或将开启糖尿病合并心力衰竭治疗的新篇章,后续研究旨在探索SGLT-2i改善心肌病变的机制,并进一步明确SGLT-2i能否为已合并心力衰竭的患者带来获益。Diabetes is one of important risk factors for diabetic vascular complication,ventricular remodeling and deteriorated heart failure.The discovery of sodium glucose cotransporter-2 inhibitor(SGLT-2i)undoubtedly marks a milestone in the current diabetes treatment strategy for diabetes and emphasizes cardiovascular benefits while lowering glucose.An increasing amount of evidence suggests that SGLT-2i have multiple cardiovascular protective effects,which correlates with the latest findings that primary cardiovascular outcomes were markedly improved in diabetic patients treated with SGLT-2i as compared with placebo.Surprisingly,researchers also found that treatment with SGLT-2i was associated with a lower risk of hospitalization for heart failure,making SGLT-2i a promising novel therapeutic target for heart failure in diabetes.Subsequent studies will explore mechanisms responsible for cardiomyopathy and further testify whether SGLT-2i could bring benefits for heart failure patients.
