摘  要:目的探讨汽油添加剂甲基叔丁基醚(MTBE)的职业性暴露对血清丙氨酸转氨酶(ALT)水平的可能影响。方法运用分子流行病学研究方法对深圳市82名在职加油站职工MTBE的暴露情况进行个体采样,并进行体格检查;以顶空固相微萃取-气相色谱/质谱法(HS-SPME GC/MS)分析血清ALT水平;以格式化调查表对ALT的可能混杂因素进行调查;以多因素logistic回归分析法探讨各因素的比值比(OR)。结果调查发现,加油站职工对MTBE的个体平均暴露水平为351.76±164.31μg/m3,尚未超过美国ACGIH推荐的阈限值(TLV)(50 ppm;即180 500μg/m3);以MTBE暴露水平≤100μg/m3组作为参照,暴露水平为100~300μg/m3及≥300μg/m3组的OR值分别为1.27(95%CI:0.87~1.44;P>0.05)及1.41(95%CI:0.89~1.68;P>0.05)。结论较低浓度的MTBE加油站职业性暴露对血清ALT水平的影响无统计学意义。Objective To explore the potential effects of exposure to MTBE on serum ALT in occupational population of petrol station attendants( PSAs). Methods The molecular epidemiology study was conducted in Shenzhen,in which the serum ALT of 82 petrol station attendants( PSAs) was measured and the personal exposure concentrations of MTBE were analyzed with Head Space Solid Phase Microextraction GC/MS( HS-SPME GC/MS). The individual characteristics potentially associated with ALT were investigated with unified questionnaires. The multivariate logistic regression analysis was used to estimate odds ratios. Result The result showed that the average exposure concentration of MTBE in PSAs was 351. 76 ± 164. 31μg/m3,which did not exceed the Threshold Limit Value( TLV) of MTBE( 50 ppm; i. e. 180,500 μg/m3) recommended by American Conference of Governmental and Industrial Hygienists( ACGIH); After adjusting for age,gender,education,current smoking habit and current alcohol consumption,the odds ratios were 1. 27( 95% CI: 0. 87 ~1. 44; P〈0.05) and 1. 41( 95% CI: 0. 89 ~ 1. 68; P〈0.05) in the groups with MTBE exposure within 100 ~ 300 μg/m3,and≥300 μg/m3,respectively,as compared to the group with MTBE exposure ≤100 μg/m3. Conclusion Statistically,exposure to low level of air MTBE in petrol station has no significant influence on serum level of ALT in human in PSAs.
