赴加移民胸片诊断与国内诊断标准的比较研究

摘　　要：目的调查福州口岸依据不同标准对不同人群进行的胸片诊断结果,探讨不同标准对活动性肺结核的临床诊断价值,提高口岸对肺结核的影像诊断率。方法收集2014年1月1日—2016年12月31日福州口岸赴加拿大移民人群的胸片资料,依据加拿大使馆颁发的《Panel Members' HANDBOOK 2013》中的胸片诊断标准进行回顾性分析,并与同时期福州口岸的出境劳务人员群体和出境海船员群体的胸片资料进行对比。结果 2014年1月1日—2016年12月31日福州口岸赴加移民人群1721人,依据加拿大使馆颁发的《Panel Members' HANDBOOK 2013》中的胸片诊断标准,胸片异常率为14.70%,其中点数1.1-3.5(minor findings)131例,占7.61%;点数≥4.0(findings sometimes seen in active TB or other conditions)122例,占7.09%,其中活动性肺结核2例。同时期福州口岸出境劳务人员4709人,胸片异常182人,异常率为3.86%;出境船员7552人,胸片异常239人,异常率为3.16%。结论依据加拿大使馆的X线诊断标准,福州口岸赴加移民群体的胸片异常率明显高于出境劳务人员群体和出境海船员群体。为提高影像诊断率,可在口岸对胸片的影像学表现设立点数评判标准进行筛查。Objective To investigatechest radiograph results among different populations according to different criteria,and to explore the clinical diagnostic value of different criteria for active tuberculosis,and in order to improve the X-ray diagnostic rate of active tuberculosis.Methods A retrospective analysis was conducted according to the X-ray diagnostic criteria of Panel Members＇ HANDBOOK 2013 issued by the Canadian embassy.The data used for analysis were collected from immigration Canadian personnels at Fuzhou port from January 1,2014 to December 31,2016,and all of themcompared with the data of outbound laborers and outbound seafarers in the same period.Results According to the X-ray diagnostic criteria of Panel Members＇ HANDBOOK 2013 issued by the Canadian embassy,the abnormal rate of chest radiograph of 1721 immigration Canadian personnels at Fuzhou port from January 1,2014 to December 31,2016 was 14.70%,1.1-3.5（minor findings）were 131 cases,accounting for 7.61%;≥4.0（findings sometimes seen in active TB or other conditions）were 122,accounting for 7.09%,of which there were 2 cases of active tuberculosis.In the same period,outbound laborers were 4709 cases,abnormal chest radiographs were 182 cases,accounting for 3.86%;Outbound seafarers were 7552 cases,abnormal chest ra-diographs were 239 cases,accounting for 3.16%.Conclusion According to the X-ray diagnostic criteria of Panel Members＇ HANDBOOK 2013 issued by the Canadian embassy,the abnormal rate of chest radiograph of immigration Canadian personnels at Fuzhou port is significantly higher than outbound laborers and outbound seafarers.In order to improve the X-ray diagnostic rate,the chest radiograph diagnostic criteria on scoring points can be established for screening.
