铁蛋白、叶酸和维生素B_(12)检测在骨髓增生异常综合征诊疗中的价值

摘　　要：目的探讨血清铁蛋白、叶酸、维生素B_(12)检测在骨髓增生异常综合征(MDS)中的临床意义。方法收集MDS患者标本87例,以体检健康者50例作为健康对照组。采用电化学发光法检测样本的铁蛋白、叶酸和维生素B_(12)水平。对3组数据进行统计学差异性分析,并计算联合检测各项指标的阳性预测率。随后,进行受试者工作特征(ROC)曲线分析。结果 MDS组血清铁蛋白和维生素B_(12)水平显著高于健康对照组(P<0.01);MDS患者血清叶酸水平与健康对照组比较,差异无统计学意义(P>0.05)。铁蛋白和维生素B_(12)联合检测对MDS的阳性预测率为77.0%,高于各项单独检测。此外,血清铁蛋白和维生素B_(12)检测辅助诊断MDS的ROC曲线下面积(AUC)分别为0.898和0.893。结论血清铁蛋白、叶酸、维生素B_(12)联合检测可以辅助MDS的诊断,有一定的临床价值。Objective To assess the clinical value of combined detection of ferritin,folic acid and vitamin B 12 in myelodysplastic syndrome（MDS）. Methods A total of 87 patients with MDS were collected,and 50 healthy subjects were selected as control group.The levels of ferritin,folic acid and vitamin B 12 were detected by electrochemiluminescence.Statistical analysis of the 3 groups of data,and calculate the positive rate of joint detection of all indicators.Then,the ROC curve was analyzed. Results Ferritin and vitamin B 12 levels in MDS group were significantly higher than those in control group （ P 〈0.01）,and there was no significant difference in serum folic acid level between the MDS group and the control group （ P 〉0.05）.The positive rate of ferritin and vitamin B 12 combined detection for MDS was 77.0%,which was higher than that of individual tests.In addition,the area under the ROC curve of ferritin and vitamin B 12 in MDS diagnosis were 0.898 and 0.893,respectively. Conclusion The combined detection of serum ferritin,folic acid and vitamin B 12 can assist in the diagnosis of myelodysplastic syndrome and has certain clinical value.
