摘  要:目的:探讨高渗氯化钠羟乙基淀粉40对颅内肿瘤手术患者颅内压的影响。方法:选取54例颅内肿瘤手术患者,分组为A、B组,在基础治疗的基础上,分别使用高渗氯化钠羟乙基淀粉40、甘露醇治疗,观察两组患者术中T0、T1、T2、T3、T4、T5阶段的颅内压(Intracranial pressure,ICP)、平均动脉压(Mean arterial pressure,MAP)、心率(Heart rate,HR)、舒张压(Diastolic blood pressure,DBP)、收缩压(Spontaneous Bacterial Peritonitis,SBP)等指标变化以及尿量;统计患者治疗有效率。结果:T0时段两组指标相近;T1~T5阶段,两组患者的MAP、DBP、SBP指标呈现下降趋势,B组波动更为明显;A组各时段ICP指标下降幅度显著高于B组;T1~T3两组HR指标均稍降,T4~T5,B组HR指标反弹提高,A组相对稳定;B组患者每时段尿量均高于A组;A组疗效高于B组(96.30%>77.78%),P<0.05。结论:高渗氯化钠羟乙基淀粉40应用于颅内肿瘤手术中,患者颅内压下降,血流动力学稳定,治疗效果更优。Objective: To investigate the effect of hypertonic sodium chloride hydroxyethyl starch 40 on intracranial pressure in patients undergoing intracranial tumor surgery. Methods: 54 patients with intracranial tumor were divided into two groups: group A (n=54) and group B (n=54). On the basis of basic treatment, hypertonic sodium chloride hydroxyethyl starch 40 and mannitol regimen were used respectively. The change of ICP, mean arterial pressure (MAPP), diastolic blood pressure (DBP), systolic blood pressure (SBP), and the effective rate of treatment were observed in the two groups during the operation of T0 and T1, T2, T3, T4, T5, and the mean arterial pressure (MAPP), diastolic blood pressure (DBP), systolic blood pressure (SBP), and etc were observed. Results: The indexes of the two groups were similar in T0 period. The decrease of ICP in group A was significantly higher than that in group B in T1-T5 stage. The HR index of the two groups were slightly decreased in T1-T3.And during group T4-T5, group B was slightly increased, and group A was not change. The volume of urine in group B was higher than that in group A, whereas the effect of group A was better than group B (96.30%〉77.78%, P〈0.05). Conclusion: Hyperosmotic sodium chloride hydroxyethyl starch 40 is more effective in the treatment of intracranial neoplasms with decreased intracranial pressure and stable hemodynamics.
