摘　　要：目的观察改良中心静脉导管在儿童胸腹腔积液穿刺置管引流中的应用效果。方法将我科2012年9月至2016年12月收治的67例胸腹腔积液需穿刺置管引流的患儿随机分为两组,观察组(32例)采用改良中心静脉导管置管引流,对照组(35例)采用常规中心静脉导管(艾贝尔)置管引流。比较两组患儿的治疗效果及不良事件发生情况。结果观察组胸腹水消失时间、导管留置时间及住院时间均短于对照组(P<0.05);观察组导管脱落次数、导管堵塞次数均少于对照组(P<0.05);两组感染、出血次数无显著差异(P>0.05)。结论改良中心静脉导管穿刺置管引流在儿童胸腹腔积液治疗中效果显著,可明显缩短导管留置时间和胸腹水消失时间,值得临床推广。Objective To observe the application effect of modified central venous catheter in puncture catheterdrainage of thoracic and abdominal effusion in children. Methods From September 2012 to December 2016, 67 childrenwith thoracic and abdominal effusion were randomly divided into two groups, the observation group （32 cases） acceptedmodified central venous catheter drainage, the control group （35 cases） received conventional central venous catheter （Abel）catheter drainage. The therapeutic effects and adverse events of the two groups were compared. Results The disappearancetime of thoracic and abdominal effusion, catheter retention time and hospitalization time in the observation group wereshorter than those in the control group （P〈0.05）; the times of catheter slip and catheter occlusion were less than those inthe control group （P〈0.05）. There was no significant difference in the times of infection and bleeding between the twogroups （P〉0.05）. Conclusion Modified central venous catheter in puncture catheter drainage children with thoracic andabdominal effusion is significantly effective, which can significantly shorten the disappearance time of thoracic andabdominal effusion and catheter retention time. It is worthy of clinical promotion.
