养阴清肺汤治疗慢性阻塞性肺疾病疗效及其对肺功能的影响

摘　　要：目的探讨养阴清肺汤对慢性阻塞性肺疾病疗效及其对肺功能的影响。方法收集我院中医科收治的慢性阻塞性肺疾病患者84例,随机分为对照组和治疗组,各42例。其中对照组予以硫酸沙丁胺醇片口服,1片/次,3次/日;治疗组在对照组的基础上给予养阴清肺汤治疗,温水送服,100 m L/次,3次/日,2组患者均治疗4周。比较2组患者临床疗效、肺功能及血清免疫学指标改善情况及中医证候积分改善情况。结果经治疗,治疗组患者临床总有效率为85.71%(36/42),对照组患者临床总有效率为69.05%(29/42),治疗组显著优于对照组,差异具有统计学意义(P<0.05);治疗后,治疗组患者FVC、FEV1、FEV1/FVC及血清IL-8、TNF-α改善均较对照组患者显著,差异具有统计学意义(P<0.05);治疗后,治疗组患者中医证候积分降低较对照组患者更明显(P<0.05)。结论养阴清肺汤对慢性阻塞性肺疾病患者可起到较好的治疗作用,具有滋阴清热、止咳平喘的功效,亦可提高患者肺功能,改善临床症状等,值得推广应用。Objective To explore the Yangyin Qingfei decoction in treating chronic obstructive pulmonary disease and its influence on lung function. Methods 84 patients with chronic obstructive pulmonary disease admitted to the department of traditional Chinese medicine in our hospital were selected and randomly divided into treatment group and control group, with 42 cases in each group.The control group was treated with salbutamol sulfate tablets, 1 tablet/time, 3 times/day. The treatment group used Yangyin Qingfei decoction on the basis of the control group warm water delivery, 100 ml/time, 3 times/day. Both groups were treated for 4 weeks. The clinical efficacy, lung function and serum immunological index improvement of the two groups and the improvement of TCM syndrome integration was compared. Results After treatment, the total clinical efficiency of the patients in the treatment group was 85.71%（36/42）, the total clinical efficiency of the control group was 69.05%（29/42）, the treatment group was significantly better than the control group, and the difference was statistically significant（P〈0.05）. After treatment, the improvement of FVC, FEV1, FEV1/FVC and serum Il-8 and TNF-αin the treatment group were significantly higher than those in the control group, and the difference was statistically significant（P〈0.05）. After treatment, the reduction of TCM syndromes in the treatment group was more obviously than that in the control group（P〈0.05）. Conclusion Yangyin Qingfei decoction has a better therapeutic effect on patients with chronic obstructive pulmonary disease, it has the effect of nourishing yin and clearing heat, relieving cough and asthma, improve lung function and the clinical symptoms, and it is worthy of clinical promotion.
