扶正补土针灸配合康复疗法治疗脑卒中后肩手综合征疗效及对肢体功能、疼痛情况的影响

摘　　要：目的:观察扶正补土针灸配合康复疗法治疗脑卒中后肩手综合征疗效及对肢体功能、疼痛情况的影响。方法:采用随机数字表法将94例脑卒中后肩手综合征患者分为观察组和对照组,每组47例。对照组患者进行常规康复训练治疗,观察组患者采用扶正补土针灸配合康复疗法治疗,观察两组患者的临床症状、肢体功能、疼痛情况及不良反应,评定两组患者治疗效果。结果:观察组总有效率为89.36%,显著高于对照组(P<0.05);两组患者治疗后中医症状积分及肩手综合症积分显著降低(P<0.05),治疗后观察组降低的幅度显著大于对照组(P<0.05);治疗后两组患者运动功能指标FMA显著升高(P<0.05),疼痛评定指数(PRI)、疼痛强度(PPI)及视觉模拟评分(VAS)水平显著下降(P<0.05),观察组较对照组上升或下降幅度更大(P<0.05);两组患者均无严重不良反应。结论:扶正补土针配合康复训练治疗脑卒中后肩手综合征能有效改善患者中医症状、缓解患者疼痛及水肿等临床症状,安全性高。Objective：To explore the curative effect of applying Fuzheng Butu acupuncture combined with rehabilitation therapy in the treatment of shoulder hand syndrome after stroke and the influence on limb function and pain.Methods：94 patients with shoulder hand syndrome after stroke were divided into observation group and controlled group by random number table method,with 47 cases in each group.The controlled group was treated with routine rehabilitation training,while the observation group was treated with Fuzheng Butu acupuncture combined with rehabilitation therapy.The clinical symptoms,limb function,pain and adverse reactions in the two groups were observed,and the therapeutic effects were evaluated.Results：The total effective rate in the observation group was significantly higher than that in the controlled group（P〈0.05）.Scores of TCM symptoms and shoulder hand syndrome in the two groups were significantly decreased after treatment（P〈0.05）,and the decrease in the observation group was significantly greater than that in the controlled group（P〈0.05）.After treatment,FMA of motor function in the two groups was increased significantly（P〈0.05）,while the pain rating index（PRI）,present pain intensity（PPI） and visual analogue scale（VAS） were decreased significantly（P〈0.05）.Changes of above indexes in the observation group were more obvious than those in the controlled group（P〈0.05）.There was no serious adverse reaction in the two groups.Conclusion：Fuzheng Butu acupuncture combined with rehabilitation training can effectively improve TCM symptoms and relieve clinical symptoms such as pain and edema in patients with shoulder hand syndrome after stroke,with high safety.
