改良助产法对减轻助产士身体负荷及分娩结局的影响

摘　　要：目的探讨半收板坐着接生对减轻助产士接生时身体受累部位负荷及分娩结局的影响。方法选取本院产科2017年5月~2018年3月自然分娩的初产妇60例作为常规组,采用传统的助产法接生;另外选取本科2017年5月~2018年3月自然分娩的初产妇60例作为改良组,采用改良助产法接生。比较两组产妇接生后助产士疲劳评定得分,产妇满意度、侧切率、产后出血、会阴Ⅲ度裂伤以及新生儿窒息、肩难产发生情况。结果常规组助产士疲劳评定得分较改良组高,差异有统计学意义(P<0.05)。两组产妇会阴侧切率、产后出血和满意度比较差异有统计学意义(P<0.05);而会阴Ⅲ°裂伤比较差异无统计学意义,两组新生儿娩出时肩难产比较差异无统计学意义,新生儿窒息率比较差异有统计学意义(P<0.05)。结论改良助产法减轻助产士身体负荷,减少职业危害,改善分娩结局,是科学的助产方案,值得临床推广。Objective To investigate the effect on load of midwifesbody affected parts when midwifes delivery, receiving half plate, sittingn the surgical stool. Methods 60 cases of rimiparas who had a baby in our hospital from May 2017 to March 2018, as the conventional group, using thetraditional method of midwifery delivery; In addition, 60 first-time mothers with natural childbirth from May 2017 to March 2018 were selected as the improvement group, using a modified method of midwifery. Compare the difference incidence of the two groups midwifes fatigue scores and pu- erperants satisfaction, side cutting rate, postpartum hemorrhage, perineal laceration Ⅲ° , neonatal asphyxia and shoulder dystocia, after delivery. Results Comparison of the modified group and normal group of midwives fatigue score after delivery, the difference was statistically significant （P〈 0.05）. Between the modified group and routine group, the difference of the side cutting rate, postpartum hemorrhage and perineal puerperaaats satisfaction were statistically significant （P〈0.05）. Shoulder dystocia incidence of the two groups was not statistically significant ifference. Perineal laceration Ⅲ° was not statistically significant difference; Neonatal asphyxia incidence of the two groups was statistically significant difference （P〈0.05）; Conclusion The midwifery is help alleviating the midwifes body load, reducing the occupation harm, improving birth outcomes. This modified midwifery program is worthy of promotion.
