肿瘤患者及其家属生命质量和善终期望的水平分析

摘　　要：目的研究肿瘤患者及家属生命质量水平、善终期望,及两者之间的相关性。方法便利抽样法抽取某三级甲等医院肿瘤患者121例,家属90名,采用欧洲癌症研究与治疗组织研发的生命质量核心量表调查肿瘤患者生命质量,采用简明生命质量量表调查其家属的生命质量。使用自行设计的问卷调查患者及家属的善终期望。结果 (1)肿瘤患者的生命质量总分为(670.02±89.10)分,为中等偏下水平。(2)家属的生命质量总得分为(40.47±3.64)分,为中等偏上水平。(3)患者的生命质量与善终期望呈一定正相关(r=0.249,P=0.006),患者的善终期望与家属善终期望呈密切正相关(r=0.407,P<0.05)。结论善终是影响患者生命质量的一个重要因素,患者和家属的善终期望相互影响。家属需要加强和患者的沟通和交流,关注患者的真实想法,并尽量满足以达到"优逝"。Objective To study the quality of life and expectation of good death of cancer patients and their families, and to explore their correlation. Methods A total of 121 cancer patients and 90 families in one tertiary grade A hospital were sampled by convenience sampling method. The quality of life of cancer patients was assessed by EORTC QLQ-C30 and that of their families by SF-12 and a self-designed questionnaire was used to investigate their expectation of good death. Results The score of quality of life of cancer patients was 670.02±89.10, which was in a moderate or below level and that of their families 40.47±3.64, which was in moderate or above level. The quality of life and expectation of good death of the patients were closely positively correlated （r=0.249,P=0.006） and the expectation of good death of the patients and their families had a closely positive correlation （r=0.407,P〈0.05）. Conclusion A good ending is an important factor that affects the quality of life in cancer patients. The expectation of good death between the patient and the families affects each other. More communications between patients and their families and more attention to patients’ real idea will be helpful to achieve a “good death” of cancer patients.
