肾移植术后1年内贫血对患者远期存活率影响研究

摘　　要：目的探讨肾移植术后1年内发生贫血(PTA)对慢性排斥反应及远期人、肾存活率的影响。方法回顾性分析自2012年1月至2014年12月收治的116例肾移植患者的临床资料。根据患者术后1年是否发生PTA,分为PTA组(n=27)和非PTA组(n=89)。比较两组患者1年内的慢性排斥反应发生率,以及人、肾存活率。结果 PTA组患者慢性排斥反应发生率为18. 5%(5/27),高于非PTA组的5. 6%(5/89),组间比较,差异有统计学意义(P <0. 05)。非PTA组移植术后1、3、5年的患者存活率及移植肾存活率均显著高于PTA组,组间比较,差异有统计学意义(P <0. 05)。结论肾移植术后1年PTA状态可明显增加移植后患者慢性排斥反应的发生率,并影响移植后人、肾存活率。Objective To investigate the effect of anemia（PTA）on chronic rejection and long-term renal/renal survival in 1 year after renal transplantation.Methods A retrospective study was performed on 116 cases of patients who underwent renal transplantation and were admitted from January 2012 to December 2014.According to whether the recipient had PTA after 1 year of operation,patients were divided into the PTA group（ n =27）and the non PTA group（ n =89）.The incidence of chronic rejection and renal survival were compared between the two groups within 1 year.Results The incidence of chronic rejection in the PTA group was significantly higher than that in the non PTA group[18.5%（5/27）versus 5.6%（5/89）],there was statistically significant difference between the two groups（ P 〈0.05）.The survival rate of recipients and the survival rate of transplanted kidney in the non PTA group 1,3 and 5 years after transplantation were significantly higher than that in the PTA group,and the difference was statistically significant（ P 〈0.05）.Conclusion The 1 year PTA status can significantly increase the incidence of chronic rejection after transplantation and affect the survival rate of human/kidney after transplantation.
