摘　　要：目的探讨自体脂肪组织中血管基质组分治疗骨伤骨不连的临床价值。方法选取月龄4~5个月的新西兰大白兔30只,雌雄不限,采用Brownlow法制备兔桡骨骨折不愈合模型。采用随机数字表法将兔桡骨骨折不愈合模型分为对照组(n=15)与观察组(n=15)。对照组给予常规治疗,观察组在对照组治疗基础上给予自体脂肪组织中血管基质组分支架材料移植。对实验兔行X线影像检查,观察骨痂出现、骨愈合时间,评价骨折愈合状况;测定实验兔血清碱性磷酸酶、钙磷含量。结果治疗后,对照组骨痂出现时间为(11. 36±0. 55)周、骨愈合时间为(35. 57±0. 73)周;观察组骨痂出现时间为(2. 86±0. 43)周、骨愈合时间为(26. 95±0. 56)周,观察组时间均较对照组缩短,组间比较,差异有统计学意义(P <0. 05)。治疗后,对照组血清碱性磷酸酶为(165. 19±23. 51)U/L,血钙磷乘积为(9. 16±1. 59)mmol/L;观察组血清碱性磷酸酶为(218. 33±36. 78) U/L,血钙磷乘积为(16. 61±5. 43) mmol/L,观察组均显著高于对照组,组间比较,差异有统计学意义(P <0. 05)。结论自体脂肪组织中血管基质组分可有效治疗骨伤骨不连。Objective To investigate the clinical value of the treatment of bone non-union with stromal vascular fraction of autologous adipose tissue.Methods Thirty New Zealand white rabbits were selected with the age of 4-5 months.The nonunion model of radius fracture was divided into the control group（ n =15）and the observation group（ n =15）by the random number table method.The control group was treated with conventional therapy,and the observation group was treated with scaffold materials of vascular matrix components in autologous adipose tissue.X-ray imaging was performed on experimental rabbits to observe callus and bone healing time,and to evaluate fracture healing status.The contents of alkaline phosphatase and calcium phosphate in serum were determined.Results After treatment,callus appeared in the control group（11.36±0.55）weeks and healing time（35.57±0.73）weeks.The time of callus appearance in the observation group was （2.86±0.43）weeks and the time of bone healing was （26.95±0.56）weeks.The time of the observation group was shorter than that of the control group,and the difference was statistically significant（ P 〈0.05）.After treatment,the serum alkaline phosphatase of the control group was （165.19±23.51）U/L,and the blood calcium and phosphorus product was （9.16±1.59）mmol/L.The serum alkaline phosphatase of the observation group was （218.33±36.78）U/L,and the blood calcium and phosphorus product was （16.61±5.43）mmol/L.The observation group was significantly higher than the control group,and the difference was statistically significant（ P 〈0.05）.Conclusion The treatment of bone non-union with stromal vascular fraction of autologous adipose tissue can significantly shorten the recovery time of the bone,which is effective in the treatment of bone nonunion.
