摘　　要：目的:探讨布托啡诺复合右美托咪定麻醉对剖宫产患者术后镇痛与性生活质量的影响。方法:选取2014年8月至2016年8月天门市第一人民医院收集的106例剖宫产患者进行麻醉镇痛处理,根据镇痛方法不同将其分为试验组和对照组,每组53例,试验组患者麻醉诱导后静脉泵注右美托咪定0.5μg/kg,对照组患者泵注等量生理盐水。两组患者手术结束前15 min均给予布托啡诺1.0 mg,术后均行患者静脉自控镇痛。比较两组患者手术后2、6、12、24、48 h的静息和活动的视觉类比量表(visual analogue scale,VAS)疼痛评分,镇痛相关不良反应发生的情况,以及采用女性性功能指数来评价患者的性生活质量。结果:两组患者在术后2、6、12、24、48 h静息和活动下VAS评分差异,均具有统计学意义(均P<0.05);试验组患者恶心呕吐发生率明显低于对照组,其差异具有统计学意义(P<0.05);试验组患者2个月和4个月的性功能评分与对照组相比,其差异均具有统计学意义(均P<0.05);7个月后两组患者的性功能评分差异无统计学意义(P>0.05)。结论:布托啡诺复合右美托咪定麻醉对剖宫产患者可以缓解术后镇痛,提高患者的性生活质量。Objective： To study the effect of Bhutto butorphanol combined with dexmedetomidine anesthesia on postoperative analgesia and quality of life in patients undergoing caesarean section. Methods： From August 2014 to August 2016,collected 106 cases of patients with cesarean section anesthesia analgesia treatment from Tianmen First People＇s Hospital,according to the different methods of analgesia were divided into experimental group and control group （n = 53）,the experimental group after induction of anesthesia,intravenous infusion of dexmedetomidine 0. 5 g/kg,control group gave normal saline infusion volume etc. 15 min before the end of surgery were given Bhutto butorphanol 1 mg. after operation were treated with intravenous patient-controlled analgesia,rest and activity2,6,12,24,48 h were compared between the two groups after the operation of the visual analogue scale （VAS） pain score,adverse effect of analgesia. And sexual function was used to assess the quality of sexual life of the patients.Results： Two groups of patients with postoperative 2,6,12,24,48 h VAS score of resting and activity had significant difference （all P〈 0. 05）; the experimental group had nausea and vomiting was significantly lower than the control group （P〈 0. 05）; 2 and 4 months of sexual function score of the experimental group and control group has obvious difference （all P〈 0. 05）; after 7 months of the 2 groups of function score was not statistically significant （P〉 0.05）. Conclusions： Bhutto butorphanol combined dexmedetomidine anesthesia for patients with cesarean section can relieve postoperative analgesia,improve the quality of life of patients.
