肝外胆管梗阻的磁共振胆胰管造影诊断分析

摘　　要：目的:探讨磁共振胆胰管造影(MRCP)诊断肝外胆管梗阻的临床价值。方法:回顾性分析2016年4月-2018年4月44例肝外胆管梗阻患者(均行MRCP检查且经手术病理/临床随访证实),并评价MRCP对肝外梗阻诊断的准确率。结果:所有患者均顺利完成检查,其中胰管和胆管树显示满意者42例,占95.45%,并且MIP三维重建图像与直接胆胰管造影片具有一定的相似性。同时,MRCP对肝外胆管梗阻的定性诊断准确率为81.81%(36/44),定位准确率为100%(44/44)。结论:临床上给予肝外胆管梗阻患者行磁共振胆管造影检查可以辅助定位、定性诊断,且具有较高的特异性。Objective：To explore the clinical value of MRCP in the diagnosis of extrahepatic bile duct obstruction.Method： During the period of April 2016 to April 2018,44 cases of our hospital with extrahepatic biliary obstructive disease were analyzed retrospectively,all were examined by MRCP,which were confirmed by surgery and pathology or clinic data.Result：All patients were successfully completed inspection,including pancreatic duct and biliary tree displayed 42,satisfied with the 95.45%,there was a certain similarity between MIP 3D reconstruction images and MRCP.Meanwhile,the diagnostic accuracy rate of MRCP for extrahepatic bile duct obstruction was 81.81%（36/44）,and the accuracy rate was 100%（44/44）.Conclusion： The accuracy rate of diagnosing extrahepatic biliary obstruction with MRI should be increased when combined with MRCP.
