培本固元方辅助卡托普利治疗隐匿性肾小球肾炎疗效及对尿C_3、24hUPQ、NGAL、β_2-MG和炎症因子的影响

摘　　要：目的观察培本固元方辅助卡托普利治疗隐匿性肾小球肾炎疗效及对患者尿补体3(C_3)、24 h尿蛋白定量(24h UPQ)、中性粒细胞明胶酶相关载脂蛋白(NGAL)、β_2-微球蛋白(β_2-MG)水平和炎症因子的影响。方法将126例隐匿性肾小球肾炎患者随机分为2组,对照组63例给予卡托普利治疗,研究组63例给予培本固元方辅助卡托普利治疗,2组均持续治疗8周。记录2组治疗前后尿沉渣红细胞(RBC)计数、24hUPQ及尿C_3、NGAL、β_2-MG和炎症因子水平,比较2组治疗后临床疗效。结果治疗后,2组尿沉渣RBC计数、24hUPQ和尿C_3、NGAL、β_2-MG水平及血清超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)和白细胞介素-6(IL-6)水平均较治疗前显著降低(P均<0. 05),且研究组均显著低于对照组(P均<0. 05);研究组临床治疗总有效率显著高于对照组(P <0. 05)。结论培本固元方辅助卡托普利治疗隐匿性肾小球肾炎疗效显著,同时能明显改善尿中相关指标,降低炎性因子水平。Objective It is to observe the efficacy of Peiyuan Guyuan decoction on latent glomerulonephritis and its effect on the levels of urinary complement 3（ C3）,24 h urine protein quantitation（ 24 hUPQ）,neutrophil gelatinase-associated apolipoprotein（ NGAL）,β2-microglobulin（ β2-MG） and inflammatory factors. Methods A total of 126 patients with latent glomerulonephritis were randomly divided into two groups. 63 patients in the control group were treated with captopril,and 63 patients in the study group were treated with Peigu Guyuan decoction plus captopril. Both groups were continuly treated for 8 weeks. The levels of urinary sediment red blood cell（ RBC）,24 hUPQ,urinary C3,NGAL,β2-MG and inflammatory factors were recorded before and after treatment. The clinical effects of the two groups were compared. Results After treatment,the levels of urine sediment RBC count,24 hUPQ and urinary C3,NGAL,β2-MG and serum high-sensitivity C-reactive protein（ hs-CRP）,tumor necrosis factor-α（ TNF-α） and interleukin-6（ IL-6） were significantly decreased in both groups（ P〈0. 05）,and the levels in the study group were significantly lower than that in the control group（ P〈0. 05）. The total effective rate of clinical treatment in the study group was significantly higher than that of the control group（ P〈0. 05）. Conclusion Peibu Guyuan decoction plus captopril is effective in the treatment of occult glomerulonephritis,and can significantly improve the relevant indicators in the urine and reduce the level of inflammatory factors.
