膀胱冲洗液温度对前列腺电切术后膀胱痉挛和出血的影响研究

摘　　要：目的观察不同膀胱冲洗液温度对良性前列腺增生症(benign prostatic hyperplasia,BPH)患者行经尿道前列腺电切术(transurethral resection of prostate,TURP)后膀胱痉挛和出血的影响,为行TURP患者的术后护理提供临床证据。方法采用前瞻性随机双盲对照研究法,将2015至2017年收治的260例行TURP的BPH患者随机分为对照组和观察组,每组130例。对照组给予室温(21~27℃)冲洗液对患者的膀胱进行持续冲洗,观察组给予加热至患者体温(35~37℃)的冲洗液对患者的膀胱进行持续冲洗,比较2组患者术后冲洗液用量、冲洗时间及血凝块堵管率与膀胱痉挛率。结果观察组膀胱痉挛率、痉挛评分均显著优于对照组(P <0. 05);观察组冲洗液用量、血凝块堵管率均显著低于对照组(P <0. 05);但2组冲洗时间比较差异无统计学意义(P> 0. 05);观察组患者冲洗前后收缩压、舒张压和心率变化值均低于对照组,差异均有统计学意义(P <0. 05)。结论行TURP的BPH患者术后应用接近患者自身体温(35~37℃)的冲洗液进行冲洗,不仅可以减少冲洗液用量,还能明显降低术后膀胱痉挛和出血风险,对改善患者预后和生活质量有更大的意义。Objective To observe the effects of different bladder irrigation fluid temperatures on bladder spasm and hemorrhage after transurethral resection of prostate （TURP） in patients with benign prostatic hyperplasia （BPH） in order to provide clinical evidence for postoperative care of patients with BPH. Methods The a prospective randomized double blind control method was used,and 260 patients with BPH who underwent TURP in our hospital from 2015 to 2017 were randomly divided into control group （ n =130） and observation group （ n =130）.The patients in control group were treated by bladder continuous irrigation with room temperature （21-27℃） irrigation solution,however,the patients in observation group were treated by bladder continuous irrigation with body temperature （35-37℃） irrigation solution.The irrigation solution dosage,rinse time,blood clot clogging rate and bladder spasm rate after TURP were observed and compared between the two groups. Results The bladder spasm rate and spasm scores in observation group were superior to those in control group （ P 〈0.05）,and the irrigation solution dosage and blood clot clogging rate in observation group were significantly lower than those in control group （ P 〈0.05）.However there were no significant difference in flushing time between the two groups （ P 〉0.05）.Moreover the changes of systolic blood pressure,diastolic blood pressure the heart rate before and after rinsing in observation group were not significant,as compared with those in control group （ P 〈0.05）. Conclusion The irrigation solution with body temperature （35-37℃） can not only reduce the dosage of the irrigating solution ,but also significantly reduce the risk of bladder spasm and bleeding after TURP in patients with BPH,which palys aaan important role in improving the prognosis and quality of life of patients.
