摘 要:目的评价重复经颅磁刺激对医务人员慢性疲劳综合征患者睡眠障碍治疗效果。方法筛选70例慢性疲劳综合征医务人员,按照随机数字表法随机分为对照组(35例)和干预组(35例),干预组行低频重复经颅磁刺激治疗,方案如下:刺激部位:右侧前额叶背外侧区,刺激强度:80%MT,刺激频率:1Hz,刺激时间:30s,刺激间隔时间:2s,总脉冲数:1 800个,治疗时间:25min,疗程:2周。对照组假刺激。在治疗前、治疗后2周、治疗结束后2周,采用匹兹堡睡眠质量指数量表评估睡眠改善效果、疲劳量表评估疲劳改善情况,不良反应症状量表评定治疗不良反应。结果干预组与对照组在改善总睡眠时间、睡眠潜伏期、睡眠质量、日间功能、睡眠效率、睡眠障碍等6方面差异具有统计学意义(P<0.05),且干预组疲劳程度较对照组减轻,差异具有统计学意义(P<0.05)。结论低频rTMS能有效改善医务人员慢性疲劳综合征患者睡眠障碍,减轻疲劳状况。Objective To evaluate the therapeutic effect of repetitive transcranial magnetic stimulation(rTMS)on sleep disorders in patients with chronic fatigue syndrome (CFS). Methods Seventy patients with CFS were randomly divided into the control group (35 cases)and the intervention group (35 cases)according to the random number table method. The intervention group was treated with low frequency rTMS given 1-Hz rTMS to the right prefrontal dorsolateral area with a stimulus intensity of 80 % MT, a stimulating time of 30 s, a stimulating interval of 2 s, the total number of pulses of 1 800, a treatment time of 25 minutes and a treatment course of 2 weeks. The control group was treated with the sham stimulation. Then, Using the Pittsburgh Sleep Quality Index (PSQI)scale to assess the sleep quality improvement effects, the FS-14 scale to assess the fatigue improvement effects and the Adverse Drug Reactions Inventory (TESS)to assess the adverse effects. Results There were significant differences between the intervention group and the control group in improving total sleep time, sleep latency, sleep quality, Day function and Sleep efficiency ( P 〈0.05), and the intervention group alleviated more than the control group on the fatigue degree( P 〈0.05). Conclusion The low frequency rTMS can improve the sleep disorders of CFS patients effectively, quickly and noninvasively, and alleviate fatigue degree.
