HER家族蛋白在结直肠癌中的表达及其临床意义

摘　　要：目的通过检测表皮生长因子受体(HER)家族蛋白在结直肠癌组织中的表达水平,探讨HER家族蛋白在结直肠癌发生发展中的作用及其临床意义。方法采用免疫组化及荧光定量PCR方法检测120例手术切除的结直肠癌组织及正常肠黏膜组织中HER家族蛋白及转录水平的表达情况,并分析HER家族蛋白表达水平与结直肠癌临床病理特征之间的关系。结果 PCR结果显示,结直肠癌组织中的HER家族蛋白基因的表达水平明显高于正常组织(P<0.05),并且HER-1与HER-3基因的表达强度与肿瘤浸润深度、淋巴转移、和TNM分期有显著相关性(P<0.05);免疫组织化学结果显示,HER家族蛋白的表达水平明显高于正常组织,其中HER-1的阳性表达率为62.5%,HER-2的阳性表达率为10.8%,HER-3的阳性表达率为85.8%,HER-4的阳性表达率为35.8%,HER-1和HER-3蛋白的表达强度与浆膜浸润和淋巴结转移显著相关(P<0.05),而与患者年龄、性别、组织学类型、TNM分期、肿瘤部位及大小均无关系(P>0.05)。结论 HER家族蛋白与结直肠癌的发生、发展可能有关,HER-1与HER-3有可能成为临床上评价结直肠癌恶性程度的指标。Objective To investigate the expression of human epidermal growth factor receptor in colorectal carcinoma and analysis the correlation of different expression levels and clinical pathological characteristics. Methods The expression of human epidermal growth factor receptor was detected by the methods of immunohistochemistry and real-time quantitative PCR （RT-PCR）in 120 freshly specimens of colorectal carcinoma and 40 normal tissues. Results The expression of HER family mRNA was significantly higher in colorectal carcinoma than that in normal tissues （ P 〈0.05）, and the expression of HER-1and HER-3 were correlated with invasive depth, lymph node metastasis and Dukes classification in colorectal cancer; the expression of HER family protein was significantly higher in colorectal carcinoma than that in normal tissues （ P 〈0.05）, HER-1 and HER-3 were correlated with invasive depth and lymph node metastasis of colorectal carcinoma （ P 〈0.05）. Conclusion The expression of HER family in colorectal carcinoma tissues exist different degree, which may be correlated with the pathological process of colorectal cancer.HER-1and HER-3 could be a potential biological marker to evaluate the prognosis of colorectal carcinoma, and may be an important target in the future.
