摘  要:目的 探讨3.0T高分辨MR的磁敏感加权成像(SWI)对高血压脑白质疏松(LA)与脑深髓静脉(DMV) 不同分级之间关系及临床应用价值。方法 80例高血压患者,其中LA 0~3级的患者各20例行常规头颅MRI、SWI检查,比较LA与DMV分级及脑内微出血灶(CMBs)相关性。结果 在80例高血压LA患者中,随着LA分级增加,DMV分级可见度降低,其中LA 1、2、3级DMV扩张、迂曲分别有2例、5例、2例, LA与DMV分级显示之间分布差异均有统计学意义(经Gamma法分析,G=-0.770,P〈0.001),LA程度与DMV分级之间呈负相关,即LA程度越高,DMV分级越低;CMBs分级随着LA分级加重,CMBs风险增加,CMBs患者占LA患者的27.5%(22/80)。结论 3.0T高分辨MR的SWI对显示DMV、高血压LA的DMV的脑内氧代谢改变和CMBs变化情况有独特的优势,有助于改善静脉血血流动力学的变化,从病因出发改善病情对老年痴呆和脑卒中发生具有重要临床意义。Objective To explore the clinical value and different classification relation between hypertension leukoaraiosis (LA) and the brain deep medullary vein (DMV) at 3.0T high resolution MR susceptibility weighted imaging (SWI).Metbods In 80 patients with hypertension,each 20 patients with LA in classification 0-3 were routinely performed head MRI and SWI.LA, DMV and cerebral microbleeds (CMBs) were compared.Results In 80 hypertension patients, with the increase of LA classification,DMV visibility was reduced.DMV expansion and circuity were showed in 2,5 and 2 cases for LA classification 1,2 and 3 groups.It has statistical significance between LA and DMV distribution difference display (by Gamma spectrometry,G=-0.770, P〈0.001).There was negative correlation between LA classification and DMV degree,the higher the LA classification, the lower the DMV grade. CMBs classification and risk were increased with LA classification improved.CMBs patients account for 27.5% (22/80) of LA patients.Conclusion For displaying the change of oxygen metabolism in LA, DMV and CMBs.It has an unique advantage of SWI at 3.0T high resolution MR, which is of important clinical significance for improvement of Alzheimer's disease and stroke.
