摘 要:目的 探讨慢性阻塞性肺疾病(COPD)患者体质量指数(BMI)与肺功能及CT体素指数(VI)之间的相关性。方法 COPD稳定期患者110例,按BMI分为低、正常BMI及超重组3组。比较各组之间BMI与肺功能、VI的关系。结果 与正常BMI组及超重组相比,低BMI组用力肺活量占预计值的百分比(FVC%pred)及第1秒用力呼气量占预计值的百分比(FEV1%pred)明显下降,残气量与肺活量的比值(RV/TLC%)及VI明显增高(P均<0.05)。BMI与FVC%pred(r=0.280,P<0.01)及FEV1%pred(r=0.268, P<0.01)之间呈弱正相关,与RV/TLC%(r=-0.230,P<0.05)及VI(r=-0.233, P<0.05)之间呈弱负相关。VI与FVC%pred之间呈弱负相关(r=-0.393, P<0.01),与FEV1%pred(r=-0.544, P<0.001)及FEV1/FVC%(r=-0.701, P<0.001)之间分别呈中度及强负相关,与RV/TLC%之间呈中度正相关(r=0.448, P<0.001)。结论 COPD患者的肺功能及VI在各BMI组别之间有一定的差异,BMI与肺功能以及CT VI之间有一定的相关性。Objective To evaluate the relationships among body mass index (BMI), lung function and CT voxel index(VI) in chronic obstructive pulmonary disease (COPD) patients.Methods According to BMI, 110 cases of stable COPD were divided into three groups, including low BMI, normal BMI and overweight groups. The relationships among BMI, lung function and VI in each group were compared.Results Compared with those with normal BMI or overweight groups, patients with low BMI showed significantly lower FVC% pred and FEV1% pred. They also demonstrated much higher RV/TLC% and VI(all P〈0.05). BMI had a weak positive correlation with FVC% pred (r=0.280, P〈0.01) and FEVI% pred (r=0.268, P〈0.01), and a weak negative correlation with RV/TLC% (r=-0.230, P〈0.05) and VI( r=-0.233, P%0.05) respectively.VI showed a weak negative correlation with FVC% pred (r =- 0.393, P 〈0.01), a moderate or strong negative correlation with FEV1% pred ( r = -0.544, P 〈 0.001 ) or FEV1/FVC G ( r =-0.701, P〈0.001) respectively, and a moderate positive correlation with RV/TLC%(r=0.448,P〈0.001). Conclusion There were some differences for lung function or VI between different BMI groups, and some certain correlations were found among BMI, lung function and VI of COPD patients.
