摘 要:目的探讨不同病理分型、症状严重程度慢性前列腺炎(chronic prostatitis,CP)患者前列腺液免疫球蛋白(immunoglobulin,Ig)、炎性因子、趋化因子水平变化及临床意义。方法 148例CP患者,依据美国国立卫生研究院病理分型分为Ⅱ型组48例、ⅢA型组56例、ⅢB型组44例,依据国际慢性前列腺炎症状指数分为轻度组49例、中度组62例、重度组37例;同期45例男性体检健康者为对照组。检测各组前列腺液IgG、IgA、IgM、白细胞介素(interleukin,IL)-6、IL-8、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、巨噬细胞炎性蛋白-1α(macrophage inflammatory protein-1α,MIP-1α)、单核细胞趋化蛋白-1(monocyte chemoattractant protein-1,MCP-1)水平,并进行比较。结果Ⅱ型组、ⅢA型组、ⅢB型组前列腺液IgG[(8.35±2.11)、(9.26±2.70)、(8.95±2.50)g/L]、IgA[(2.95±0.37)、(2.88±0.34)、(3.11±0.43)g/L]、IgM[(2.83±0.21)、(2.72±0.46)、(2.92±0.48)g/L]、IL-6[(0.32±0.17)、(0.36±0.16)、(0.23±0.06)μg/L]、IL-8[(0.37±0.17)、(0.34±0.14)、(0.21±0.13)μg/L]、TNF-α[(0.70±0.26)、(0.71±0.26)、(0.55±0.15)μg/L]、MIP-1α[(215.35±20.48)、(181.26±16.89)、(155.76±14.62)ng/L]、MCP-1[(352.66±33.67)、(287.68±29.74)、(240.47±32.68)ng/L]水平均高于对照组[(2.35±0.27)g/L、(0.49±0.17)g/L、(0.34±0.13)g/L、(0.14±0.08)μg/L、(0.17±0.02)μg/L、(0.44±0.15)μg/L、(131.3±10.95)ng/L、(202.43±21.53)ng/L](P<0.05);Ⅱ型、ⅢA型、ⅢB型组前列腺液IgG、IgA、IgM水平比较差异无统计学意义(P>0.05);Ⅱ型、ⅢA型组前列腺液IL-6、IL-8、TNF-α、MIP-1α、MCP-1水平高于ⅢB型组(P<0.05);Ⅱ型组前列腺液MIP-1α、MCP-1水平高于ⅢA型组(P<0.05),IL-6、IL-8、TNF-α水平与ⅢA型组比较差异无统计学意义(P>0.05);轻、中、重度组前列腺液IgG[(7.06±1.21)、(10.31±1.86)、(15.81±3.06)g/L]、IgA[(2.37±0.73)、(3.92±1.04)、(5.89±1.30)g/L]、IgM[(2.48±0.62)、(3.57±0.90)、(4.97±1.11)g/L]、IL-6[(0.28±0.09)、(0.3Objective To investigate the changes of immunoglobulin (Ig), inflammatory factors and chemokines in prostatic fluid of patients with chronic prostatitis (CP) and their clinical signit:icances. Methods Totally 148 patients with CP were divided type Ⅱ group (n= 48), type ⅢA group (n= 56) and type Ⅲ B group (n= 44) according to the pathological classification proposed by the National Institute of Health, and were divided into mild group (n = 49), moderate group (n= 62) and severe group (n= 37) according to International Chronic Prostatitis Symptom Indexes. Another 45 healthy volunteers were as controls (control group). The levels of IgG, IgA, IgM, interleukin (IL)-6, IL-8, tumor necrosis factor-α (TNF-α), macrophage inflammatory protein-1α (MIP-1α) and monocyte chemoattractant protein-1 (MCP-1) were detected and compared among groups. Results The levels of IgG ((8.35 ± 2. 11 ), (9.26 ± 2.70), (8.95±2.50) g/L), IgA ((2.95±0.37), (2.88±0.34), (3.11±0.43) g/L), IgM ((2.83±0.21), (2.72± 0.46), (2. 92±0.48) g/L), IL-6 ((0. 32±0.17), (0. 36±0.16), (0. 23±0.06) μg/L), IL-8 ((0. 37±0.17), (0. 34± 0.14), (0.21±0.13) μg/L), TNF-α((0. 70±0. 26), (0. 71±0. 26), (0. 55±0. 15) μg/L), MIP-1α((215. 35± 20.48), (181. 26±16.89), (155.76 ± 14. 62) ng/L) and MCP-1 ((352.66±33.67), (287. 68±29. 74), (240.47± 32.68) ng/L) in type Ⅱ, Ⅲ A, and ⅢB groups were significantly higher than those in control group ((2. 35 ±0.27) g/L, (0.49±0.17)g/L, (0.34±0.13)g/L, (0.14±0.08) μg/L, (0. 17±0.02) μg/L, (0.44±0. 15) μg/L, (131.3±10.95) ng/L, (202.43±21.53) ng/L) (P〈0.05).
