快速补液扩容在高龄患者麻醉中的临床分析

摘　　要：目的:探讨快速补液扩容在高龄患者麻醉中的临床应用效果。方法:随机抽取2016年1月~2017年1月某院收治的高龄颌面部手术患者共96例,按照随机数字表法,将患者随机分为研究组与对照组,对比两组不同时间段血流动力学变化、麻醉前后心肌缺血发生情况及麻醉并发症发生情况。结果:麻醉前10min、拔管后15min,两组血压、心率对比,结果无统计学意义(P>0.05);插管时、切开后、拔管时,研究组血压、心率均低于对照组,结果有统计学意义(P<0.05);麻醉前后,两组心肌缺血发生率对比,结果均无统计学意义(P>0.05)。所有患者术后意识均清醒,均未出现明显麻醉后并发症。结论:高龄手术患者麻醉时实施快速补液扩容的效果更为理想,能促使血流动力学处于稳定状态,且不会增加心肌缺血和麻醉并发症发生风险,值得进行深入研究和推广。Objective： To investigate the clinical effect of rapid rehydration expansion in senile anesthesia. Methods： 96 senile patients with maxillofacial surgery admitted in a hospital from January 2016 to January 2017 were randomly selected. According to the random digital table method, 96 cases were randomly divided into the study group and the control group. The occurrence of myocardial ischemia and the complications of anesthesia were compared between the two groups. Results： After 10min before anesthesia, 15min after pulling tube, blood pressure and heart rate were compared in two groups, results were not statistically significant （ P 〉0.05）. The blood pressure and heart rate of the study group were lower than that in the control group when intubation, incision and catheter was cut, and the results were statistically significant （ P 〈 0.05 ）. Before and after anesthesia, the incidence of myocardial ischemia was not statistically significant （ P 〉0.05）. The postoperative consciousness of all patients were clear and there was no obvious postoperative complication. Conclusion： The effect of the operation in senile anesthesia with implement of rapid rehydration expansion is more ideal,it can lead blood flow dynamics in a stable state, and does not increase the risk of myocardial ischemia and anesthesia complications, worthy of in-depth research and extension.
