Assessment of carotid artery elasticity by echo tracking technique in rabbits with carotid plaques

摘  要:目的应用回声跟踪(echo tracking,ET)技术定量评价颈动脉斑块模型兔的颈动脉管壁弹性,探讨斑块形成后颈动脉管壁弹性的改变,并与脉冲多普勒技术的诊断价值进行比较。方法对43只健康雄性新西兰兔在第0周及高脂饮食第4周末分别行右侧颈动脉超声检查,测量并记录二维超声观察的阳性结果、脉冲多普勒技术血流动力学指标及ET技术参数的数值。血流动力学指标包括:搏动指数(pulsatility index,PI)、阻力指数(resistance index,RI)、收缩期峰值流速(peak systolic velocity,PSV)、舒张期末流速(end diastolic velocity,EDV)及平均血流速度(mean velocity,MnV)。ET技术参数包括:动脉僵硬度参数(stiffness parameter,β)、压力应变弹性系数(pressure-strain elastic modulus)Ep、动脉顺应性(arterial compliance,AC)、单点脉搏波传递速度(one-point pulse wave velocity,PWVβ)和脉搏波放大指数(augmentation index,AI)。根据第4周末二维超声下斑块的检测结果将43只实验兔分为2组:无明显斑块组(A组)和斑块组(B组),比较各组间检测结果的差异。结果在第4周末所测得的所有弹性参数值分别与第0周比较,β、Ep及PWVβ均较基础状态增高,差异有统计学意义(P<0.001);AC、AI及脉冲多普勒各指标差异无统计学意义。B组各参数与A组进行比较,β、Ep和PWVβ的数值显著高于A组,差异有统计学意义(P<0.05),AC、AI及脉冲多普勒各指标差异无统计学意义。结论 ET技术既可用于检测形态学正常血管的早期弹性功能异常,也可用于评价动脉斑块形成后动脉弹性的改变,其对斑块形成后动脉弹性功能异常的诊断价值优于脉冲多普勒技术。Objective To investigate the carotid artery elasticity in rabbits with carotid artery plaques by echo tracking (ET) technique, and compare its diagnostic value with pulse Doppler technique in assessing vascular stiffness. Methods Forty three healthy male New Zealand rabbits were fed with high cholesterol diet. All rabbits were examined by 3 ultrasound methods at the beginning and the end of the 4th week in this experiment respectively, which were two dimensional uhrasonography, pulse Doppler technique and ET technique. The hemodynamic indexes were detected by pulse Doppler technique, including pulsatility index (PI), resistance index (RI), peak systolic velocity (PSV), end diastolic velocity (EDV), and mean velocity (MnV) ;the elasticity parameters of carotid were measured by ET technology, including stiffness parameter (β), pressure strain elastic modulus (Ep), arterial compliance (AC) ,one point pulse wave velocity (PWVβ) and augmentation index (AI). According to the results of plaques at the end of the 4th week, the rabbits were divided into 2 groups: no plaque formation group (group A) and plaque group (group B). All datum detected at the beginning were defined accordingly as group A0 and group B0. Then,comparing all datum among these groups. Results Elasticity parameters β,Ep,and PWVβ increased significantly at the end of the 4th week compared with those at the beginning (P〈0. 001). The same results were found between group B and group A, showing parameters β,Ep, and PWVβ of group B were higher than those of group A. No difference was found between AC and AI, as among the references of pulse Doppler technique. Conclusions ET technique can be used in detecting carotid artery elasticity with or without plaque, and its diagnostic value is better than pulse Doppler technique.
